Well it seems like in this alternate timeline Korekiyo's sister is still alive. The Mikan event with him implies that she's bedridden and was interested in having Mikan nursing her (Until he saw through her), and the Hifumi event seems to imply that he finds Hifumi's interest in incest disturbing. So maybe he's not sick in the head here. Who knows.

I just like that Danganonpa the main twist about Korekiyo's was dude who looks and acts like a serial killer actually IS a freaky serial killer I think that is alluded to in V3. It kinda fits in to the whole "game being a giant troll to the fans" feel the game
