I thought I heard you say in an earlier video that you made a house call to an elderly lady? If so, you are absolutely incredible! I didn't think ANY doctor made house calls anymore. That would make you unique. Love your videos. Thank you for sharing them.❤

Hi, great video, thank you. Your patient must have felt such relief after you cleared her ear. Interesting story of your journey to becoming a nurse. You got further than most in your musical career, mine didn’t get beyond singing into a hairbrush in front of the mirror!! Thanks for sharing.

Fantastic job. You really do need to go slow because the patient is already in pain from the Keratosis Obturans. Neel Raithatha does many of these and he’s very good too. He uses the hood to slice through not always successful and then uses the Zoellner. But great job Nurse Richard.

Keratosis obturans is very complex and very interesting for us on lookers!!! Lol ... i enjoy this and choosteotoma very much even tho i feel terrible for the patient that has to endure these issies ... but alas, great videos sir!! Much love from Connecticut USA 🇺🇸

This is the first time I've see Keratosis Obturans removed without using anything besides suction. Usually, other tools are needed to remove the skin. Nicely done!

On the other hand, that's why this particular procedure took the good nurse so long; an ear hook, such as a St. Bart's, very likely would have shortened the time significantly! However, Richard, in this episode, does not strike me as being in his best form; almost like he has a cold or the flu. Bottom line, he got the job done very satisfactorily👍.

I’ve seen a lot of videos where olive oil, medical grade, is used to soften and help the skin to release. You do an excellent job!!

@No More Ear Wax I always hang on to a good analogy or visual aid that helps me grasp a concept.. especially when it is something you have to explain frequently! I'm a nurse in the USA and you guys on YT make me want to uproot, move to the UK, and become the next hot thing on the wax scene 🤣 Not even joking, though!

I finally got a chance to watch the video. I can only imagine the relief the patient felt when that last big piece came out. I didn’t see any bleeding. Would call this stage 1?

More than likely, was a bit of delicate/red tissue so borderline 2, will know more when i clean it out fully next time 👍

I watch all the ear wax removal videos and really enjoy seeing the different styles each audiologist brings to their job. I commend you all, very professional and highly qualified. I don't understand people that comment telling you which tools you should have used or how you should have approached the situation. Seriously each audiologist has their own technique but they are all experienced in their field and I'm pretty sure they don't need advice from people who watch the videos and think they know better. If you dont like a video scroll on past, why take the time to comment something negative. Some people were obviously never taught that if they haven't anything nice to say they should keep quiet 😂 Anyway great job, really enjoyed the video and how gentle and patient you are.

I always think of keratosis obturans as being like a ball made of rubber bands - every new layer of skin is like adding more rubber bands to the outside. Individually they're thin and soft and flexible, but when they're packed tight enough they're solid as a rock.

​@@thewaxwizardI agree a great analogy. I have always wondered Richard with this much content in the ear is the patient not able to feel it with a finger or would it not be seen with the naked eye. With that have you ever had a patient whose ear was so full that it had had begun to empty out into the cup of the ear. Just some wEARd questions from a new wEARdo.😜🤪🥸👂Havingbthe very best time catching up on missed videos .🥳

What causes keratosis obturans?? Is there some preventative measure one should take to make sure this doesn’t happen to them??

I don't think the exact cause is known, other than a failure of the skin to migrate out of the ear as it normally would. So you cant prevent it i dont think.

One thing that can interrupt migration of skin is when the ear canal is grazed or scraped by digging around in there, like with a cotton bud/w-tip. We were taught nothing in your ear but your elbow or nothing smaller than your elbow.

Whether it hurts or when it starts to feel good to them? And, like, when you’re doing skin peels or suctioning infection debris? What does that feel like to them? Thank You! Keep up the good work. 👍😀

Did the patient have any severe itching? And once you remove the skin, does it help stop the itching?

I sometimes use sodium bicarbonate drops for skin. And sometimes i watch but rarely! I just ramble on and then click post!

@@thewaxwizard I meant sodium bicarbonate! Sorry, calling it wrong one but was in regards to getting the skin to release. I’m fine with the rambling lol. Video though, I have subconscious movements. You will notice in most early videos Rhys held his hands together. We like it when people speak with their hands, but when people read lips it helps if they’re out of the way. Accessibility tip for deaf or hard of hearing. 😊

That was a tough one, that you handled very well. In a lot of videos out there, it seems like the person doing the removal is much too concerned with the dramatic effect. Pulling out as big a lump as possible. In those videos there's a lot of "going around the elbow to get to the thumb", if that makes sense 😆 Seems to me that you don't do it that way. That patient care IS your main concern,whether it comes out in a lump, or goes in the tank. I applaud that. 🌞(excuse the rambling)

Probably grade 1 or grade 2. Difficult to say until you get the other stuff out and look at the canal wall. But I'm amazed you that much out with suction. Often forceps and hook needed on these too!

This might actually look like it was fighting back! I've seen others use an instrument that looks like a spatula. It's flat and it slips around the sides really easy. I thought it would have been handy for this one! Do you have either that flat one or one that looks like a spoon?

because if you go too firm, the patient experiences pain. Its a very painful condition, and it's always best to go slowly. This type can't be rushed.

Tried it early on, but was too uncomfortable, it was too tightly packed against the canal wall, no gaps to get it in behind.

Poor work, using only suction, takes a lot of time and causes serious discomfort, Change the technique of extracting the keratosis mass, use a different tool! 2/10

The patient felt no discomfort at all, some plugs are too deep for tools. I'll try to get a 3/10 next time! 😀

The suction doesn't cause serious discomfort. Shoving tools into an ear canal already chock full of wax and skin would cause serious discomfort. Your silly rating is utterly supercilious. If you don't have anything kind or constructive to add, then your comment only serves to be rude.
