This shows how truly badass Duskdude is: guy was put on meathooks, both shoulders pierced, about to be butchered by leathernecks, and what does he do? Get off those hooks by himself, pick up sickles and take on 3 crazies with chainsaws, and come out on top. All with both shoulders being severely injured. He deserved to become worthy (I think the sad part though is that he didn't really want any of this).

The end text for Episode 1 describes Duskdude's shoulders as "still throb[bing]" from the meathook wounds, meaning he went through everything in that episode with those injuries. Hell, given he jumps immediately into each episode without any breaks and the environment designs suggesting the whole game happens over the course of only a day or two, those wounds were probably still pretty fresh by the time Duskdude fights Nyarlathotep, and that's ignoring all the other injuries he'll undoubtedly take during the game as well as re-piercing himself on the meathooks at the end of As Above, So Below. "Worthy" doesn't even begin to cut it.
