Kotoamatsukami can't be spammed. It's considered as the perfect genjutsu because its target doesn't even realize they're under its effect. Such a powerful genjutsu comes with an obvious drawback, a massive cooldown time. This is why Danzo was hesitant about using it while fighting Sasuke. He wanted to use it on Tobi/"Madara". If the wielder of Shisui's eyes happen to have EMS, then maybe it might be spammable to a certain degree.
