The International Patient Safety Goals (IPSG) were developed by the World Health Organization (WHO) and implemented by healthcare organizations around the world. They are a set of guidelines that aim to improve patient safety and reduce the risk of medical errors. The current version of the IPSG includes the following goals:
1. Identify patients correctly: This goal emphasizes the importance of verifying the patient's identity using at least two patient identifiers (such as name and date of birth) to ensure that the right patient receives the right treatment.
2. Improve effective communication: Effective communication is essential in healthcare to ensure that information is accurately shared among healthcare providers and patients. This goal emphasizes the need for healthcare providers to communicate clearly and effectively with each other and with patients and their families.
3. Improve the safety of high-alert medications: High-alert medications are those that carry a high risk of causing significant harm if used in error. This goal aims to reduce the risk of medication errors involving high-alert medications by using standardized processes for ordering, dispensing, and administering these drugs.
4. Ensure correct-site, correct-procedure, correct-patient surgery: This goal aims to prevent wrong-site surgeries or procedures, which occur when a procedure is performed on the wrong part of the body or the wrong patient. Healthcare providers are encouraged to use a standardized process to verify the correct surgical site and patient before the procedure.
