As an expert on the subject and in particular as an #OFA (Opioid Free Anesthesia) fan, I have defined in more than one situation that #OFA as the best #OBA (Opioid Based Anesthesia) technique. I state this not only because of the redundancy of the expression, but essentially to highlight the importance of opioid drugs in anesthesiology. Of course, strictly speaking, they are the only intravenous anesthetic drugs. Understanding a general anesthesia without opioids will only be possible by understanding the importance of what we want to eliminate. In this way, the entire OFA vs OBA comparison, whether in terms of the “Pro vs Con” debate or as a comparative study, is of course condemned to what is defined as scientific noise. Without taking these premises into account, studies of the superiority of one technique over the other are clearly destined to what is considered failure and to confrontation, given the impossibility of comparing "apples" with "oranges" and the significant derived bias. As always, both OFA and OBA have more in common than what separates them.
