Heistern is located on the northern edge of the High Fens-Eifel Nature Park. To the east flows the Wehebach. The village is located on a district road between Hamich and Hücheln and near the country road between Schevenhütte and Langerwehe at 200 m above sea level.
In the immediate vicinity is the Wenau Monastery, Holzheim Castle, Wenauer Höfchen, the hamlet of Blumenthal, the ruins of Karlsburg Castle, Schönthal Manor, Kleinschönthal and Pützmühle. A hiking trail leads to the Laufenburg.
