Amazing quality both in terms of presentation style and quality of content. As a 64 year old I am presbyotic, but have been wearing corrective lenses for only two years. I have had a couple of really poor experiences with opticians here in UK and as an ex commercial pilot my curiosity of all things technical led me to educate myself. John, you very kindly responded to a couple of my emails over the last year and I repeat my thanks to you for taking the time to do so. If I was younger and had a chance at another career, I would definitely go into this profession - it satisfies so many aspects of what makes me tick. Quite likely my favourite video thus far, I cannot express how much I'm learning just from your videos, so I thank you, John! But I do have a question: when marking up progressive lenses, the fitting cross should be at (or just below) the patient's pupil, if I remember correctly, and should be marked up as such, yes? I've asked around at work just how to mark up office lenses (which I know are limited to just intermediate and near, and have no distance) and whether they should be dropped a tad or not as they have zero distance... Nobody seems to fully know, and I can't find a definitive answer online, so I'm asking here - should probably be in an email, but this works as well. I ask, because just a few months ago, I embarked on a new path and began my career in opticianry and am wanting to challenge the licensing exam, sometime this year; and your videos are certainly helping with that.
@LaramyKOptical
@LaramyKOptical 4 жыл бұрын
You need to find out which specific office lens you are using and find the fitting guide for that lens. Different ones have used different height measurements (and even PDs) over the years. Most likely spot on center of pupil. I wouldn't think you would want to drop it since you are already working with a lens that is set up for only int and near. John

Found the charts, now just to tackle the different types of anti-fatigue lenses, their individual fitting heights, etc. My experience in the lab has given me a decent amount of insight into fitting, and I'm learning how to mark em all up now. I have astigmatism (around 2 dioptre) and with my last 2 sets of progressives for computer work I've been unsatisfied with the alignment to my eye, and distortion when I look at different places on the screen without turning my head. Its coding, so a lot of screen time staring at text, which needs to be crisp to avoid eye strain. I think Ill go single vision for computer work . Would a digitally surfaced lens be able to improve the distortion over the whole field at a fixed distance? Also, there is a lot of ambiguity in commercial offerings - as I understand it, you might get conventional optical measurements (ie better/worse) with digital surfacing, or even 'wavefront' measurements (if that's the correct current term, IDK) with conventional surfacing. What language do I need to use to say that I want the best metrology, and the optimal computed and surfaced lenses for the application? And how do I find an optometrist and a lab that will provide that (because most front of office staff wont even know, and will bamboozle you with another 10 trade names)?

Can you discuss about the difference and benefits of dual surfaced freeform vs front surface freeform vs back surface freeform??
@LaramyKOptical
@LaramyKOptical 4 жыл бұрын
With very few exceptions (actually none that I know of) all progressives are a back surface. The front surface may be quite special and unique, like Camber, but it is still a factory or pre-molded front design. All the true free-form surfacing takes place on the back of the lens. Yep, I know... it seems like you should be able to take any raw chunk of plastic and whip up a perfect free-form lens, but that just isn't how it works. The equipment all still operates on a front base-curve process. Drop me an email through the OpticianWorks website and I'll follow up on this some more. Great question. Might make great video! John

I had cataract surgery but am having trouble with glasses. I got some over the counter readers but they are like looking thru fog and are hard to clean. I bought progressive from the local optical company ( not the eye surgeon as he does not prescribe glasses) but same thing. I think the problem is the type of lens … poly carbon ( I think that is the lens type) Are there any other of material that lenses are made from? What are my options?
@LaramyKOptical
@LaramyKOptical 2 жыл бұрын
Hmmmm... I'm guessing that your vision without glasses is not like looking through a fog? How long ago was the surgery? [fog = edema = give it time] Your follow up doctor should provide you with [should have provided you with] a new prescription. That may well be as simple as an over the counter pair - but - it may not. I wouldn't think that a certain lens material would appear foggy and hard to clean can be a problem with low-end non-glare but two pairs? To answer your question Trivex or even old fashioned CR-39 for lower powers would be the alternatives to polycarbonate. And either go with a high-end non-glare or no non-glare.

Hello, do you think Zeiss's SmartLife technology makes sense even on monofocals? They advertise it as such (it improves vision for anyone, from people "who don't wear glasses yet" up to any prescription), but I suspect that, if your near vision isn't blurry and troubled, their "near-vision helping" technology may just contribute to make the adapting muscles of the eye lazier and lax sooner than they would otherwise get.

Correct, for goodness sake if you don't need glasses then don't wear glasses. Looks like SmartLife is just their marketing buzz word for free form lens design.

I have worn progressives for 25 year with no issues - I was a -8.00 with +2.75 add with .75 cyl in L eye. BUT I had cataract surgery which left me at about -1.00/-1.25 with .25 cyl in both eyes. I need about +2.50 add. Is it possible to find something that might work given that I have lost the ability to focus? I have tried a few and am having difficulty. Can people use progressives successfully after cataract surgery? It seems that my long eye exacerbates all visual issues. When I am corrected for distance my intermediate vision is obliterated. And with no correction I can see the computer well and read all but the tiniest print. My intermediate and near vision are very close. Any ideas?
@LaramyKOptical
@LaramyKOptical 2 жыл бұрын
Short answer #1 is: A progressive should be the answer. Or, yes a progressive is often used after cataract surgery. It provides the variable power that can replace what has been lost. Short answer #2 is: Buy a bunch of cheap over the counter readers at 1, 1.25, 1.50, 1.75 and experiment with what works for what you are doing at intermediate/near. Once you know the "perfect" power consider having a good pair made up. Short answer #3: Don't overlook an old fashioned lined bifocal! It sounds like you would go a tad weak on that full 2.50 add to hit the sweet spot. I feel like I could go on for an hour ---- look around for a good independent optician, one willing to talk and listen and work with you. John

What’s a good progressive lens for someone with a balance lens (one good eye)?
@LaramyKOptical
@LaramyKOptical 2 жыл бұрын
Not being funny here - the one that works best for you. It will depend on what you do, how you use your glasses and lens powers. A "balance" design (not about one eye) but a lens that tries to balance all the ranges might (might) be best but hard to say. Do your best to find a great optician, maybe one that sells lenses made by IOT. Of course they will carry on about "safety" for the one good eye. Trivex as a material would be a great idea - but no guarantees on safety/impact resistance unless you have a safety frame/lens. Also if your lens power is quite high the lens becomes safety thickness or thicker anyway. You should save some money by doing a SV lens in the non-sighted eye! Don't overlook those second pair glasses for any hobbies. John

Free form doesn't seem to be widely available in safety glasses. Is this because you have less control over the base curves than a conventional varifocal?
@LaramyKOptical
@LaramyKOptical 2 жыл бұрын
Most likely a matter of cost. If you are buying them you can get a free-form - when the company is buying them - probably not. Certainly nothing to do with base curve choices. If a company allows progressive safety then a free-form would only be better.

Oh and it dawned on me in the middle of the night - forcing thickness to make a lens safety might negate some of the benefits of free-form where the goal of ultimate thin/thick is part of the design software. I just watched a few of your videos and learned that you're very likely a "gun guy" and also that you know lenses. My my 61 year-old eyes and I have already discovered that progressive lenses are completely unsuitable for shooting. I'm thinking that a traditional (lined) bifocal with a modest NV-add for my right (dominant) eye might work well, if I can "flip" the NV area it to the top of the frame (exactly the opposite of what you need for reading, because a good cheek weld on a rifle stock puts the line of sight to the front sight near the top of the lens). Where can I find a forum to chat about the options? No one seems to know much of anything at all (especially optometrists and opticians)when it comes to shooting sports corrective eyewear! I think you should do some videos specific to shooting sports.

Cal, LOL actually sold off the entire collection shortly after making that video! I think I bought a new stereo and went back to listening to vinyl with the money. Anyway check this out: kzbin.info/www/bejne/q4OYfWOXisqdjMU There people that know how to design lenses for shooting. John

@@LaramyKOptical Thanks so much for your prompt reply, John! After watching the video you linked, I know what to ask when phoning a new potential optician ("free form", "double 'D'", "O.C.", "occluders", etc.) I've been asking eye professionals for years and didn't know about the stuff you cover in your videos until I stumbled upon your channel. BTW, I have hundreds of vinyl LPs and an old direct drive turntable with a Grace tone arm. I really want to get a quality tube RIAA preamp!

John, I just sent a request for Laramy-K customers/ Optician contacts in my area to your CSR and I copied you. Thanks! I studied physics and even optical mineralogy at University and then became an Electrical and Computer Engineer. Your videos here make a world of sense to me! -Cal

@@calbrabandt5967 I saw that! You may also try the two places I mentioned at the beginning of the video about occupational lenses. They may well be willing to work with you online - which considering where you are might be your only option anyway. Keith, the K in Laramy-K is an engineer and knows his physics too. I'm a bear of little brain but between the two of us we make it work. I do OK, he makes sure it passes engineer standards. And you lot are a tough bunch! John

I would like to see the video about why you are not a fan of those specific measurements in the future!
@LaramyKOptical
@LaramyKOptical 4 жыл бұрын
I'm not sure it is video worthy... I'm not a fan of of POW measurements. I could go on for hours here but I'll try to keep it short --- 1) If you are an experienced confident seasoned optician that knows what they are looking for and know what and how to measure the POW points, well, go for it. 2) IMO until we have a tablet or similar based system that holds BOTH the capture device AND the human head in a stable, repeatable position you are simply measuring against nothing. If I can move the measuring device and I can move my head it all becomes meaningless. 3) You are talking about trying to capture degrees of angle with two objects capable of moving 5, 10, 15+/- degrees at any moment in any position. A human head and set of human hands. 4) The default measurements used by the software are statistically accurate. Sample sizes well above the need for 100% accurate data sets. 5) When we insert a set of actual lenses into that frame every single angle and vertex will change with them. That data you captured either by hand or by tablet - it's now about as good as that default data provided by the software. 6) Just a SWAG on my part but I'd be willing to bet a lot of money that if you had 50 opticians that claim they can do great POW measurements and you had all 50 do them the results would be pretty scary.... then take those results and make them into actual glasses and compare them to the defaults - I bet the defaults win. And, yeah I know the current thinking/argument is that, "Without POW measurements it's not really a free-form." And to that I say poppycock. And anyone insisting on that belief has a pretty poor understanding of what a free form lens is all about. Short answer is: We simply don't have the right tool for the job yet.

(Spoiler alert, HE DOES NOT, IN FACT TELL YOU WHICH PROGRESSIVE LENS IS THE BEST AS HIS TITLE SUGGEST) 22 minutes of him rambling on about something only another optometrist would get excited about. Useless for a regular person looking for a pair of glasses

You know, it's a great video, but you're 100% right. I understand that there might not be much difference between progressive lens sources now, but with this title, there should be some coverage of manufacturer claims. I mean, I read something like this review of the Varilux X-Fit and it does sound like there's a legit difference: www.iexplain.org/review-varilux-x-best-lenses/ Is the corridor honestly wider? Is the slightly less clear distance vision the trade-off? What does the "Play-Doh" tell us about wider corridors? Which brand name lenses make this trade? Which patients would benefit from wider-corridor, marginally less clear distance lens? That is, if I'm fitting someone, how can I tell who is more likely to appreciate this trade-off & say, "These are the first progressives that don't feel like they're trying to throw me into a ditch," and who is going to slam the glasses down in two weeks and say, "I can't see with these!"? You can even tell me about Laramy-K's algorithms -- is corridor width a variable for your lenses? Why or why not? There are certainly classes of trade-offs and companies who lean into each with their offerings. None of that is covered here though the title suggests they would be.

Very interesting video. I have been using the so called 'HD Progressives' and paid according $CAD450 just for lenses, and now need new ones. According to your Putty examples, these unavoidable 'distortion cylinders' appear to be around 40-50% of the total lens area for a 'do-it-all' progressive. Studying the marketing brochures for new lenses, however, if you believe the hype, they appear to be able to reduce these distortion cylinders to approx. ~

@LaramyKOptical 7 ай бұрын
There is no "magic pixie dust." Assuming they are just a year or two old the HD lens you got last time is about as good as it got and about as good as it can get. I'd look to getting the same again. Sadly if you spent down and didn't like them you can't get that money back and then you have to spend up on top of that. Yes - the marketing brochures are all BS illustrations. Any gains these days are infinitesimal.

You just saved At least $400 for me. It is very difficult for a layman to understand this kind of thing. They're fundamentally foolish and that is the reason they got over board expectations. During my practice of 30 years as a doctor I have understood this very clearly. I am wearing single vision lens for near and far in two pair of glasses after cataract surgery. And that gives me great vision You are an excellent teacher. But to understand what you are telling requires basic knowledge of physics and anatomical moments of the eye. I have got a temptation to go for so called progressive. Then I searched. Now I got the answer. Thanks.
