using D100 (and by extension d12) can cause obstacles to be rerolled into trapdoors. As of repentance trapdoors no longer naturally spawn in Black Markets or I AM ERROR rooms (replaced by a void portal). But before that you used to be able to use teleport 2.0 and the Battery to infinitely loop the void

The thing that happened in The void is pretty much what would happen if you took a trapdoor that would usually lead you to the next level in Chest/Dark room in Rebirth. Theory is that the game does create a new floor as if you went down the trapdoor, but since you are in the last chapter, the only type of floor that can be generated is Chest/Dark room and void in this case.
