i think it is a animation issue, the logic of the charger is charge, hit a survivor the survivor get Attached to the chargert as a animation of being carried, if it hits a wall it should stop and start the punching animation that puts the survivor on the ground, during the start of the punch animation the game is very unique with it sounds, everything have specific soundtracks and light, before the animation its a white focus light , during the punch its a orange light with a different soundtrack only for that specific part of the attack, you can see the survivor get teleported once the light gets to orange with means the charger switched the animation to the punch one, and for the game the cahrger was still "carrying nick" somehow the nick did hit something, and the charger did not stop or do the animation of stopping when it hits a wall, causing a full desync on the animation, you can see thsi is also very wrong cause that orange light NEVER turns on, if the charger is in the AIR because it needs to hit something, so something in teh game did go really wrong, the charger was supposed to stop at that hit but a bug in the code of the game did not,. left 4 dead and valve games in general have amazing soundtracks and very detailed details, theres specific sountracks that play on every Stance of the animation, ^for example the smoker have the sountrack when its pulling you and another soundtrack when its starting do damage, as well as the light changes that i call "Focus light" with is essentially a invisible light that only appear for the plaayer so you can see you character very well during the attacks, the light change as well, and with the charger thing you can see very clearly the orange light with is when it switch to attacking and again it should NEVER do this in the air the game as treating like if the charger was not in the air.

I think it's one of those ladder bugs. I'm only guessing here It's easier to do on longer ladders, like on the No Mercy Finale, but I think that if an object is falling towards you, (Say, a molotov or a spitter's spit, but i don't know why zombies don't count) while you are climbing a ladder, it accelerates you downward really fast and you die to fall damage.
