When your health reaches 0 from hanging on a ledge, the Survivor is forced into a "falling" animation where they will keep taking damage and move slowly. They are still able to shoot and use health items until they inevitably die.

It is possible to do this on some custom maps, where there is a ledge above deep water and falling into it will not kill the Survivor (see fact #47). However, you would need to use pills in mid-air as you would have 1 HP left from the ledge and would die instantly otherwise.

Play left 4 dead style mutation. You ll get ragdoll if you die
