Never tried realism. It's neat to see how something as little as removing the outlines for survivor changes how the game work. What would be lower priority targets like jockeys and smokers are full ordeals now.

It definitely makes the game a lot harder. Without being able to see your teammates through walls and from far away, you can't easily identify teammates that are in trouble, so you have to rely on audio cues and pure communication to find them. Item outlines are also removed unless you're really close to them. Other things that change are: -Common Infected take MUCH less damage from body shots (0.5x on Easy, 0.37x on Normal, and 0.25x on Advanced and Expert). You're gonna want to go for headshots if you want to be efficient with your ammo. Or use a melee weapon or the Magnum. -No rescue closets. If you die, you're out of the game until your team finishes the chapter, they wipe, or someone brings you back with a defibrillator. -The Witch is INCREDIBLY dangerous on Realism. She does not take extra damage from point-blank shotgun blasts, meaning you can't crown her. Additionally, on any difficulty other than Easy, she instantly KILLS you if she hits you. She also has the same body shot resistance that Commons have. Your only options are to ignore her completely, or fight her and PRAY she doesn't get you. And just like the tagline for Realism Versus states, the Survivors can't see each other, but the Infected can still see THEM. This heavily skews the balance towards the Infected, making for a much more intense and challenging game.

Because infected team already set up around the witch to provoke it immediately. Come close to Witch means you go straight to the trap of infected.
