reactivity is no more indicated in Shunts with elevated PVR ! it is only indicated in idiopathic PAHTN to look for Ca++ blockers responders ( 7-10 %).

i want to ask about measuring ESPAP through VSD ??? is it accurate as actually we may have VSD with max PG across 30 mm HG and TR with gradient across about 35 and by adding RA pressure which is 5 ................ so by TR , ESPAP is 40 only and by VSD it's expected to be 60 supossing systolic systemic pressure is 90

In absence of PS, TR is a good estimator for PASP providing that you have a good envelope. In the mentioned scenario, mostly the TR envelope was not complete (underestimated).
