Doctor किसी भी पैथ के हों उनके शैक्षणिक डिग्री सर्टिफिकेट के साथ ,आधार, क्लीनिक का भाड़े का एग्रीमेंट या खुद का जमीन पर बना है तो उसके कागजात रेवेन्यू रशीद के साथ अपने जिला के सिविल सर्जन ऑफिस से जमा कर क्लिनिकल इस्टेब्लिशमेंट का प्रोविजनल certificate पा सकते है , जिसका मामूली बैंक ड्राफ्ट के द्वारा फीस देना होता है।
