Here's a very obscure mechanic you overlooked: While you do get your bar refilled at the end of each stage, if you keep full health on your current bar you get your previous one back. At least, that's what I remember hearing and witnessing when others played it.

You just need to press F2 when using Mame. I don't know if it's a different button in other emulators. This is something that can also be done in the real arcade ports, although it is accessed in a different way.
