It's horrible, honestly it's basically the complete submission of your health. Most sub/dom relationships are fine with consent and generally leave short term damage when pain is inflicted. Its really well written because clearly she isn't "a big fat person" but in the movie he has some sort of body dysmorphia going on so that's what he says to Foster's character when she asks - not that the girl was the daughter of a politician or the woman on the news etc. The movie, imo at least, wasn't calling her fat, and she clearly wasn't, even plus-sized is a little overboard I would say.
