The ESO data always will be based upon the agency's ability to have providers document accurately. Defining first pass, ecouraging employees to document acurately without worry of discipline, and further training and clarity of documentation are all variables when extracting this data.

Absolutely. Obviously, this applies to any electronic record system that is abstracted. Basically, every retrospective dataset that didn’t do independent data observation. We have to consider this when evaluating all papers… how was the data collected? Great point!
