It's a matter of self assessment. The pilot must feel that he/she is at his/her best to perform duties of a pilot. Insomnia is a contradicting condition that might not be acceptable for pilots. In case a pilot has not been able to get sufficient rest, pilots can request for a sick leave due fatigue. Only when its appropriately applicable. Pilots are given with sufficient rest time in between two flights. Any irresponsible behavior that could affect line operations and safety of the flight is unacceptable in aviation. Doing so could certainly have grave consequences such as license suspension/losing job etc. This is not a medical opinion. Please refer to the respective company policies in the subject matter.

Good question!! In India RTR exams are not being conducted since sept 2021. Neelay & Winged engineer both have our DGCA exams cleared, FAA license & medicals FIT but we still need RTR exams to be conducted in order to get Indian DGCA CPL. So, unfortunately the question you've asked might be a question for WPC.
