I just had the LAL+ March 5th and get my first light adjustment tomorrow. I can already see 20/20 distance, intermediate and very good for near. Amazing depth of field…amazing. I have a small astigmatism 1.25 that will get corrected and some tweaking. I am excited. No halos, excellent night vision.

Thank you for sharing your experience. It’s great this you have so good results, which is not typical for monofocal, you are lucky. I assume that you have the LAL in both eyes, and the target refraction is a bit different, which makes a monovision resulting in that depth of field effect. And small amount of astigmatism may add it as well. Please share your experience after the adjustment. Wish you all the best!

⁠@@iol-adviserLAL+ is a new IOL that has built in range of vision prior to any sort of treatment. Highly likely there is no "monovision" in this instance.

My results are about the same although my monovision suffers a bit while driving at dawn and dusk. I can still see clearly but I get just a bit of blur when looking at bright lights. For the the most part my brain tends to focus on the eye that is seeing the best ie; right eye for distance and left eye for near. I really don't even have to think about it. If I did a lot of night driving, I would get some Rx glasses or contacts to adjust my near, (left), eye for driving from sunset to sunrise.

Alexei, Thank you so much for the review! I've been waiting for something new for a long time (As you may remember). I still haven't put in my lenses, the excitement and stress about my eyesight is gone, I'm using glasses for now..... I guess I'm used to it.

In my opinion, LAL should be the standard of care for cataract surgery. I had a small amount of astigmatism. Was very near sighted. Had surgery in May 2024. Had 3 adjustments. I now have Excellent intermediate and distant vision. But need to wear glasses for reading. Achieved 20/15 in both eyes. I am very happy. I have never had such good vision. I wore contacts for 60 years.

I thought that with LAL you can only adjust it once. Anyway, it's very sad, there are so many options, but none of them come close to natural lenses which are able to focus instantly at any distance.

Yes. We have that limitation. We are not able yet to clone all the human body parts or organs.

@@iol-adviser I choose a toric. With my astigmatism there's a fair chance I won't need glasses for driving and distance activities. The adjustable would make that more likely but the cost difference is just not worth it. I've been wearing glasses since 2'nd grade so I'm used to it. Also for driving I almost always wear sunglasses. Uncorrected distance would be a plus, but not a great advantage. A cost benefit analysis. Thanks for your videos.

This was a very good explanation, and I will probably end up with one LAL lens soon as I only have a cataract on my right eye. I had Lasik surgery over 20 years ago and my eye doctor recommended the LAL lens but it is $5000 !!!

Doc my eyes have myopia -1250 both eyes I go to Thailand for treatment But they say my eyes have high pressure in the eyes Which is not suitable for ICL lense input or laser treatment ! It is say for me I need stick to holy glasses, is there any treatment for myopia for my case I am tried of glasses

hi! if ICL is not an option, and laser as well - the only solution to get glasses off is a refractive lens exchange - to a PC IOL (presbyopia correcting lens)

I have had LAL for more than a year, but why does the distance my eyes see change? Is it not locked? After the first three months, I didn’t need to wear glasses to read. Now I can’t see without glasses. Why is this happening? can you tell me? thank you

if the lens is locked, but you see changes in distance you need a diagnistic to find out what is going on with the eyes and to define an action plan if needed. that's optics, and if there is a change it might be lens dislocation, astigmatism and so on

What is the best way to reduce refractive surprise post cataract surgery with monofocals like eyhance? Does ORA use during surgery helpful?

ORA will not help too much. Precise calculation is number of factors all together - measure, calculation and follow up. When I’m doing lecture on this topic it’s about 2 hours talk :)

Thanks for the helpful information. I didn't know it was only monofocal. My Tecnis synergy toric lens implants have been a disaster. YAG laser made it worse - daylight to dusk extremely blurry. Options left - wear prescription for a while and if successful , LASIK. Otherwise lens extraction and replacement with another lens. I want multifocal, my eyes were considered very healthy, just having astigmatism. I only had first day after surgery - beautiful vision. 2nd day blurry and worse. Ten months later PCO evident in eyes and wrinkling over lens, which is when I got YAG. My ophthalmologist can extract lenses and replace them with another multifocal lens (difficult procedure after YAG due to posterior capsule burnt open from laser). But now they say they need to make sure my visual system is compatible with multifocal lenses, EDOF lenses, trifocal lenses. Otherwise a monofocal lens and glasses for the rest of my life, which is why I paid so much money to avoid.

Thank you for your comment and sharing your sad experience. I am really sorry to hear that.. This seems to be a case of missing the refractive target. The best option, if they are ready to exchange the lens is to go with EDOF theoretically, but the particular model depending on your eyes refraction (axial length to be more precise), cornea status in terms of astigmatism and visual needs.. we may have a deeper discussion of the options in a consultation (I'm available from the next week). A suggestion to you - ask a direct question what exactly happened with your visual system that it might be "not compatible" the FRV/ERV (full range of vision a la trifocal / extended range of vision a la EDOF), that sounds strange to me. good luck!
