Well here in the carribean batty boy means a gay man but its like a slur. I pretty sure it originated from jamacia but I not sure. If I were to call somebody a batty boy I is calling them gay.

Search for "batty boy song" and you'll see a couple songs by well known artists that are all about shooting gay men. (I myself couldn't listen for more than about thirty seconds before getting too disgusted to go on.) Lil Nas X seems to be taking the slur and leaning into it in the gayest and most fabulous way possible (not surprising), and topping it off with changing the gun to a metaphor for dick/cumming. I love him so much.
