You can look into advance care planning, which allows you to name a "substitute decision maker" to act as your representative when you are no longer capable of making medical decisions. I was actually watching a Hong Kong informational video introducing this. You may have to research this on how to make that document legal and enforceable. Furthermore, advance care planning is recommended for anyone over 18 because you can write down beforehand how you want to be cared for while you are capable of expressing your wishes. It's like a will document but for your medical care.
