The dead child is representing their future children they won't have. Then you have the part where He says Marshall get a grip that his true self trying to stop what's about to happen before it's too late. The song is being performed by his alter ego Slim Shady. The song is absolutely beautiful in representing the pain of cheating but the struggles of remaining yourself when you use an alter ego. Think of it like actors portraying the Joker. The get deep into the dark side of that character and have trouble leaving that behind when the movie is over. Jack Nicholson warned Heath Ledger about his struggles doing that.
