As an officer, he has to hear, for himself, the victim express those feelings. It’s part of the report. Especially if the victim is going to press charges.

The question if he was in fear of his life is a standard question to justify the state statute for that criminal charge. The question about if he felt pain was not for the state Statute itself but standard to determine if emergency treatment is needed by ambulance or the hospital (or both) based on the victims statement.
