Locally Advanced Breast Cancer

In this talk, we discuss evidence for management of locally advanced breast cancer. We review indications for postmastectomy radiotherapy, including in the context of neoadjuvant chemotherapy (NAC). We also discuss nodal management including SLNB after NAC and role of internal mammary nodal irradiation.

Addendum regarding adjuvant therapy if incomplete response to neoadjuvant chemotherapy (around minutes 25-28): For hormone positive, Her2 negative breast cancer with incomplete response, preferred adjuvant therapy is aromatase inhibitor + CDK4/6 inhibitor (Abemaciclib) per MonarchE trial which compared hormone therapy + CDK4/6 inhibitor vs. hormone therapy alone.
