The story is set back in the past during the plague when blood transfusions weren’t fully developed and discovered yet, so when the blood transfusion was a success, instead of thinking of it as a new helpful discovery in medicine, they thought of it as a way to cheat death because the ill person should have naturally died. That’s why they believe it to be a sin.
