Dear Professor Rudnicka! I absolutely adore your videos! You have the best trichology and dermatology channel on KZbin! Dear doctor, could you please tell me if you are accepting new patients? If yes, how can I book a consultation with you? I’m dealing with androgenic alopecia and would be really grateful to have an amazing doctor like you look into my case. Thanks a lot!

So interesting and well explained! I started 2.5 3 months and all was right, just last days noticed, in some part of the day, like a chest pression discomfort but: no tachicard, no edema, no heart rate. I went to a cardiologist and I took: ECG, Stress Test and echocardiogram and all was ok, so strange: also noticed, a day, the chest pain was increasing or decreasing moving body to a specific side: left/right: Any hint or suggestion? Thank you

*@profLidiaRudnicka* *1. Soo Professor are you stating the oral is more effective compared to topical due to enzyme conversion to active form? what if this mechanism is activated in the topical route to avoid systemic side effects? what would you suggest is the best activated topical formula that gets the best of both world? ie topical application without the side effects of the oral but only the benefits of the oral by converting it to the active form?* - as don't see any other reason to take oral minoxdil for hair loss, if there is do let me know... *2. minoxidil lowers diastolic BP and that in turn can cause the heart rate to increase to compensated for the loss in pressure hence its not suitable for the heart long term, any thoughts on mediating against this? - what can one do to counteract this?* *3. also whats your thoughts on other advanced mesotherapy solutions that trigger regrowth and stimulation of the hair follicles that are proving superior to PRP and Polynucleotides etc?* *PS: I have found a few companies willing to compound the topical to the active form*

The study indicates that the efficacy is comparable. What I was saying was that topical hair/scalp treatments have many variables that are difficult to control. For example, the same dose will have a different biological effect in a person with long hair compared to a shaved scalp, because of a different penetration to the scalp. 2. Minoxidil decreases the blood pressure by an average of ~3 mmHg. That is considered biologically non-relevant. There is no proof whatsoever for your conclusion. If this would be correct, then every person with hypertension would have bradycardia and every treatment for hypertension would be toxic. 3. Mezotherapy = intradermal injections. Thus, the injected substance will finally also have a systemic effect. My conclusion is that we should not inject substances that were not investigated in clinical trials and are formally approved. This being said, yes, I believe that in some diseases (e.g. androgenetic alopecia) a direct approach via intradermal injections could be beneficial. However, we should not be misled by the concept that for mezotherapy we do not need to have any systemic safety concerns.

Dear Professor Rudnika, Thank you for the video. I was looking for the answer about stop using the minoxidil, but couldn't find the answer in your video. I am considering to take 0.25 mg of minoxidil for hair loss due to stress. But I would like to know how challenge it would be if I want to stop it after a while?

I have used topical finsteride before and it didnt have any side effects as such but I want to switch to oral for better results.

A compound pharmacy do capsules with minoxidil powder inside. Do you think I should ask another form ? (Sublingual or something else ?)
