In the manga, for the final clash Sasuke and Naruto just barely graze each other. Naruto scratches the headband, and Sasuke grazes his chest. Afterward, Naruto faints from fatigue and Sasuke was coughing blood and feeling side effects from CS2. I don't think Sasuke could've killed him either way.

This is a great point, but reading the comments here I see people always forget this is a story, and the writer has to translate their message through actions within the story. Naruto while technically being right about how Sasuke's actions were wrong, didn't realize his own actions were bad too. Naruto wanted Sasuke to be reasonable, without offering actual help. Naruto had Kurama, Jiraya, and much more support, while Sasuke didn't have much. He was taking for granted how Sasuke was suffering, Sasuke while being talented and being an Uchiha, didn't have anyone else. Orochimaru while clearly lying and being a terrorist, kept proving what he preached, he told Sasuke, if he wanted more power he knows where to go, and proved it, Kakashi couldn't stand up to Orochimaru, and neither did Jiraya or Tsunade. Sasuke had nowhere to go. Naruto thought he had the right answer but he only had the right concept. Every important fight in Naruto goes like this. Mind game is just as important as the fight and Naruto had no game at all.

He then decided to leave him alive when naruto was knocked out. They both chose mercy, but if both were bloodlusted, it would be a double death. That's also narratively consistent.

The answer is alot simpler and was even confirmed in their second fight. Naruto is trying to capture and bring him back alive so he is holding back. If he was just ready to kill him or wanted to then the fight would have been completely one sided.

Always pissed me off that he lost that battle and wondered how he just scratched his forehead when he could've hit the torso. Back then I assumed it was because he was hit by Sasuke first and thus missed his aim and if you look at the clip carefully you'd see that so I think it wasn't fully intentional from Naruto to aim for the headband. It could also be as you said a reference to their previous discussion on the hospital rooftop. Only Kishimoto could confirm either.
