Thank you so very much for this video! Active 38 year old man here, and mine occurred after a new squat routine. I had the bladder pain, urgency, and frequency. I went to the urologist three times, all urine tests and prostate tested fine. Suffered for over 2 years and I felt so alone about it. Strenuous weight-lifting combined with an office job is not a great combination. Thank you so much for spreading awareness to know that there's hope to get back to normal!

We would recommend being seen by a pelvic floor pt in addition to the others you've seen! They can help with lifting mechanics and options to help the sit pain (if it's uncomfortable sitting long periods in the office).

I am on my 5th urologist now. I refuse to accept that I will have to deal with erectile dysfunction for the rest of my life. My penis was working perfectly well prior to varicose vein surgery and then the surgery did something to my body to not only cause ED, but Testicle pain, urinary leakage, hip tightness, bone pain in my legs, numbness and tingling in my arm, tennis elbow, back pain etc. It has been 18 months since surgery and none of the many specialists and urologists and primary care doctors that I have seen can figure out what is causing any of my symptoms and have not fixed one symptom. 5 urologists and all of have to show for it is 5 prescriptions to Cialis!!! That is all doctors do, hand out drugs like they are candy. But zero solutions bc they can't actually fix any penis problem or most medical conditions. Now I see why so many people say doctors are useless. Urology needs to find a cure for erectile Dysfunction already. Even if the pharmaceutical companies had a cure, they would not release bc they are making way too much money with Cialis and Viagra. Every man on the planet will need their drug at some point. Why would they give that cash cow up by coming out with a cure?? ED has ruined many young men's lives including mine and urology has Absolutely nothing to offer them but a temporary drug that comes with side effects. How about figuring out how to surgically correct the blood flow to the penis? How about figuring out what causes a man to stop getting erections and lose sensitivity and fix it? Men with ED have to do their own medical research to fix their ed bc a urologist is useless to men with penis problems.

Thank you for creating this video. Now when someone asks me what I’m going through I can send them this video. I am happy to talk about my condition to anyone that asks but it helps to have a professional explain all this. Often times it can be frustrating when someone doesn’t understand the concept of how muscles in the pelvic function, or when someone tries to gaslight my condition. Awareness needs to be spread, that is for sure. To any man reading this, you are not alone. Don’t be afraid to ask for help and be vulnerable. That’s just the first step in the road to recovery. 👍

​@@tommac21 And have them give you antibiotics and the run around. The second uriologist I saw got it, he gave me and alpha-blocker and told me to do yoga. The first gave me antibiotics, she knows a lot more compared to most uriologist. Were all upset with this carp, second time for me.

I know I'm echoing the sentiment of every other man in the comments, but thank you for making this. I've known the basic nature of my condition for more than a year now but having this video describe my symptoms on point and confirming it as hypertonic muscles is extremely validating. I've had two urologist shrug their shoulders at me but am currently seeing my second physical therapist on a routine which is helping me gradually recover, though I still feel like I'm missing pieces to the puzzle which is why I'm exploring content like this

So has going to a physical therapist helped you actually?? I’ve had this condition for almost 3 years. One day it just kind of stopped HOWEVER recently it showed it’s ugly ass face again and has me in a world of DISCOMFORT!!!! And I don’t know what to do

Bless you for these videos. 25 year old male was diagnosed with interstitial cystitis back when i was 13. Had bladder pressure and frequency for these 12 years and now i believe reverse kegals, sitting on a tennis ball, and relaxing these muscles ease my symptoms. I'm going to make these relaxation exercises my daily routine. I think a giant reason so many people get IC is because of our sedentary lifesyle, office jobs and sitting all day long tightening those muscles, and some of us it just screws up our normal bladder and bowl function due to the tightness. This knowledge is so rare for people to find and we need to get this stuff out there, especially for the poor guys like me that got diagnosed with IC or men diagnosed with porstatitis but its really just screwed up pelvic floor muscles. Thank you for what you do!

I cant pass tools a make a lot of efforts even i had diarrhea which i should make a lot of efforts. And my prostate swell every time i finish my toilet. Please anyone had this symptom talk to me

I developed this condition 2 months ago thinking it was urinary track infection. It's a long story with the doctor testing me and found blood in the urine and test me for STD which came back negative. I was pretty much left on my own trying to figure out what's wrong with me. This was when I pretty much turned to Google and KZbin. I eventually thought I had prostatitis and was leaning towards visiting a urologist but after watching bad outcomes with that condition and realized I may have to live with this condition for the rest of my life. I pretty much keep searching for answers until I stumbled on pelvic floor disorders which is often misdiagnosed as prostatitis. I did the exercises which helps alittle but not satisfying. I did more research and I eventually realize that one of the symptoms that caused the condition is dieting which I remember i started a low calestrol diet around the time i started feeling the first symptoms of my condition. A month prior, my doctor recommended i start a low calestrol diet because my blood calestrol level was high. Realizing this i decided to stop the diet by eating a Wendy's meal for lunch knowing that its rich in calestrol and I remembered talking a short nap and waking up and realizing wtf the pain was 90% gone. After constant pain for over a month. Only when I am asleep the pain is gone and during my waking time the pain would come and for my first waking time the pain was gone. All I can say from my experience is this condition can be dietary related. I did a quick research on calestrol role in the body and realized that it's pretty much important. I was thinking calestrol was bad for my health all this time not realizing how important it is. My research on calestrol also mentioned that all of our hormones especially our sex hormones is calestrol base so probably I would theorized that probably an hormone is responsible for relaxing the pelvic muscles and when this hormones can't be produced it causes pelvic pain. The human body is still a mysterious place.

Over 15 years I have suffered off and on with CPPS. It began in 2005 with what I thought to be a urinary tract infection. It took a severe bout in 2007 for me to diagnose myself as having prostatitis, a course of Cipro seemed to cure that.... little did I know that Cipro is also anti-inflammatory. After another flare up in 2010 I was convinced that my recurrent 'prostatitis' was purely inflammatory. Finally after another 3 month bout this spring I have discovered CPPS and pelvic floor disfunction. Now I know after all these years what has destroyed at least two relationships, lost me a job and at times seriously threatened my mental health. I have stopped taking antibiotics and used probiotics to restore my gut flora and recently begun a series of stretches which are helping lower inflammation and the tightness of my pelvic floor plus lowering my nervous tension. Thanks to you for further helping me to understand this horrible syndrome and to do what I need to keep it under control and hopefully eliminate it from my life!

@@PelvicHealth Thankyou for that link, the over use of flouroquinalones is near criminal. I feel badly let down by my primary care provider, the NHS. I have seen a physical therapist, my gait was poor because muscles in my left hip were so tight that my left leg was made shorter than my right, my whole pelvic area is tight, further work both by the therapist and stretches will help that. I will be seeing a pelvic floor therapist in future, hopefully in about two months.

Thank you for this video. Any other men out there that are distance runners that have been diagnosed with Osteitis Pubis and/or Prostatitis that have struggled for a solution. It took 6 months of not running last time to start training and competing again. I did 19 half marathons in 13 months and all the pain hit hard one day and it's been 3 months with very little progress.

Two months ago I had a kidney stone that sent me to the ER. I had the normal urge to urinate associated with a kidney stone which I passed the next day. But my constant urge to urinate never went away. I went to my uroligist and he ran the gamut of test on me and could not find anything wrong. They wrote it off as a pinched nerve in my back because the MRI showed a very slight bulging disk. I went to a PT and they worked with me but she was the first to tell me it may be a pelvic floor issue. She is moving me to a PT who specializes in pelvic floor issues. Fingers crossed. This video gave me great hope because the symptoms and issues I have are very close to what is described here. Any info or comments from the video producer or other viewers are greatly appreciated.

Passing a kidney stone is a painful event that can cause pelvic floor issues, which in turn can create urinary urgency, frequency, and pain. The pelvic floor physical therapist should be able to help you and we wish you a speedy recovery!

Thank you for this video. And thank you for responding in the comments. My issues started a year and a half ago. Random pelvic pain, usually stabbing. Mostly in my public hair region. Sudden onset of frequent urination with burning. Went to the doctor because I thought UTI. Checked ok. Had pelvic ultrasound twice, and everything looked "normal". No hernia either. Pain continued off and on. Sometimes worse than others. 8 months ago, after w pretty bad case of Covid my PSA # jumped. And jumped again 3 months later. Urologist thought it was a fluke. His examination showed a normal size and feel for my prostate. Retested PSA 3 weeks later and PSA came back down. Now I deal with random stabbing pelvic.l pain, urination stream is much weaker than before as well as ejaculation force. Visited a pelvic floor therapist locally. Unfortunately they don't take my health care insurance and I can't afford $150 per visit so I stopped going. I do some stretches but it doesn't seem to help sometimes painful. Please answer this part.. Is it normal to feel more pain after kegel type exercises? It is really scary to me. I feel at a loss. I don't know what to think.. Thank you!!!

We're not surprised you feel pain after kegel exercises, but it's not normal. It's a sign that something is not working correctly. The pain is occurring because there is muscle dysfunction, and doing more kegel exercises will likely make it worse. Those muscles need rest. You could practice resting them with Yoga for Male Pelvic Pain kzbin.info/www/bejne/eH_Ih5muipWUfKc Your stretching routine should not make your pelvic pain worse. Your description of stabbing pain in the pubic hair region makes me think the genitofemoral nerve is irritated. This could be irritated from an imbalance in the muscles around your pelvis. You may want to get a second opinion.

Really informative and thanks for putting this video together. I think it may be a good idea to mention that these things are often not spoken about regarding men and can feel embarrassing. I think recognizing this may bring other men into the fold and give them social permission to visit you and not feel quite so alone or humiliated. They would only be coming to you if they had a problem in the first place. Talking about that problem is another hurdle to get over. Talking about that problem with a younger, attractive female is almost impossible for an older man. Just entirely too many psychological and cultural hurdles to get over for an older gent.

@TheAvera-I too was diagnosed with IC yrs ago after rejecting a prostatitis diagnosis from my original urologist. I too had chronic constipation and a history of weighted squat exercises and sitting and stress at work. I recently lost my wife with obvious resultant stress. Since initiating male pelvic floor stretching seen on KZbin, my symptoms have 100% resolved. 🙏🏻 My guess is that it was stress and tight pelvic floor structures all along. That is why Urologists sometimes prescribe Valium suppositories to relax the pelvic floor structures. This video explains CPPS the best I have seen.

I am actually seeing this video because I have been suffering for years now, episodes of pain and urinating frecuently pain in the bladder, inside the rectum after poop, it is a real pain in the ass this condition, sometimes goes smooth others are rough but I definitely will end this freaking pain which I know is curable, we will get through we are not alone, I have seen urologist, proctologist, and even physio but all have been uncertain.

In 2018 I pulled my oburator internus and it caused pudendal nerve pain that mimiced prostate pain, uti pain, bladder pain, the whole nine. Pelvic floor PT and pudendal nerve block injections was the treatment. It took them over a year to figure it out. Took two years to start getting better.

How your oburator internus could be pulled? Is because of body exercise like gym/body building too much? What are the symtoms? Did your genital area feel numb? Or like sitting on the tennis ball like you are sitting on stranger thing?

I’m 28 and the pain I feel makes me think I have some sort of colon cancer. But it affects my bladder and peeing as well. It’s much more likely to be this right? I was sick with a food virus weeks before and these symptoms have lingered. I have a desk job and then go to the gym too. It just makes sense. I have IBS as well. But this annoyance impacts my life so much. Completely honest, it feels like my sphincter is always tight.

We are sorry to hear about the pain you are experiencing. Pelvic pain can truly be life altering. Pelvic pain can present with a variety of symptoms and often times, bowel, bladder and sexual dysfunction. IBS or other diagnoses are highly associated with pelvic floor dysfunction and pelvic pain as well. We highly recommend to find a specialist in your area to evaluate your case and and develop an individualized plan for you which may include appropriate referrals to other specialists in addition to PT.

Thank you for this very enlightening, straight-forward video. I'm experiencing pelvic pain and sometimes it does feel lonely and lost, but I do believe/ feel like i'm improving with a lot of stretching exercices and meditation. In the video, you don't mention pain in the gluteal muscles as frequent symptoms, can they be related? I started experiencing pain in the area a couple of months after tension in the perineum began.

I'm glad you feel like you're improving. Yes, the gluteal muscles can also be related. if you're not doing so already, it would be best for you to consult with a pelvic floor PT to fine tune your exercise routine at home to continue to progress.

Thank you for the video. I been dealing with this problem about 1 year. I ignored it about 11 months. I’m 26 years old. I went to see Urology 3 days ago. Test results came back negative. Now my Doctor referred me to Pelvic floor physical Therapy. I’m ready to go and make an appointment. This pain is burning,inflammation me every day. It just doesn’t feel right. I feel like my penis was about to fall off. My anus is burning n swelling. This is the worst pain of my life! 😪 it bothering me so much while I work. Sometimes it changing my behavior because of the pain I felt. I really hope I’ll be back to normal. I can’t take this pain anymore. 😢 I never have this type of pain before and it’s so confusing. I can’t imagine having this pain the rest of my life 24/7 I’ll be dying. I really do not understand English that much when doctor describe me what cause my pain and I was so confusing. I can’t wait to see PT so they can help me my life back to normal. I appreciate the video it help me a lot.

What I can say is do some exercise, get out your comfort zone, dont let it get to your head a lot of it is in out head and when we get anxious and nervous and start overthinking about it we make it worse believe me. I been smoking a lil weed also that has been working along with just trying to keep going or busy

For the past month I've been dealing with constant burning and burning when I pee - like having a constant UTI. At the same time I've been having pain in my tailbone. I went to the urologist and they found trace leukocytes in my urine and diagnosed me with bacterial prostatitis and have been on antibiotics ever since. I've been back to their office 2 more times because my symptoms are still not better and each time they still show trace leukocytes and want me to stay on the antibiotics. I found that my pain would come on usually late morning and then progress throughout the day and be worse at night. I would sleep fine and when I woke up I would feel great and then the process would start all over again. I'm a fairly big guy - 6'2", 230lbs and my job requires me to sit a lot and I also sit quite a bit when I get home. I also have poor posture and tend to slouch when sitting. I found that by sitting less that my symptoms have started to greatly improve. I hardly have anymore tailbone pain and the burning is mostly gone. I'm wondering if I've been dealing with pelvic pain this whole time?

4 years ago, I got an episode where I was going to the bathroom every 3 minutes just to try to urinate and nothing came out. I thought I had prostate issues, it was horrible, I could not sleep cause I had to be goin' to the toilet to pee constantly I was so Misérables, one night I thought about doing the unthinkable. I went to the urologist and all prostate test came negative. I found out about this issue by accident on KZbin, my doctors didn't know what was going on. I bought an electronic muscle stimulator, and heat pad and a Homedics massager. I did exercises to relieve the muscles in my pelvis and in just a couple of days I was getting relieved. Funny think is as I started to get better I started to get stabbing pains and muscle twitches in my rectum that went away a week after I did the exercises.

I've been going through this, and it's more complicated than just stretches. You have to ask yourself why is your pelvic floor muscles inflamed. A lot of has to do with your nutrition, your diet plays a big role in inflammation. If you're eating a lot of processed foods and they have a lot of seed oils those are very inflammatory to the body. Are you experiency leaking gut? Your stress also plays a big part. Are you being active throughout the week? Liver issues can also cause inflammation of the pelvic floor muscles and throughout the body as well. It's not just the muscles that one has to work on. It's very complicated. But nutrition is definitely important I would suggest also to start intermitting fasting as well. If you're overweight, I suggest you lose weight. Over masturbation can also cause these issues. Got to give your body time to heal and rest. And make sure you're at least getting your 8 hours worth of sleep.

Sadly I went to the therapist for sometime with no relief and they finally said they didn't think they could help me so I have no idea where to go from here. I have heard of botox injections but not sure they are approved at this point so insurance might not cover. My symptoms are extreme pain 95% of the time during the night. They disappear for a month or more but always come back some times as much as 3 times a week and even 2 to 3 times a night

Thank you for this video. My issues started a year and a half ago. Random pelvic pain, usually stabbing. Mostly in my public hair region. Sudden onset of frequent urination with burning. Went to the doctor because I thought UTI. Checked ok. Had pelvic ultrasound twice, and everything looked "normal". No hernia either. Pain continued off and on. Sometimes worse than others. 8 months ago, after w pretty bad case of Covid my PSA # jumped. And jumped again 3 months later. Urologist thought it was a fluke. His examination showed a normal size and feel for my prostate. Retested PSA 3 weeks later and PSA came back down. Now I deal with random stabbing pelvic.l pain, urination stream is much weaker than before as well as ejaculation force. Visited a pelvic floor therapist locally. Unfortunately they don't take my health care insurance and I can't afford $150 per visit so I stopped going. I do some stretches but it doesn't seem to help sometimes painful. Please answer this part.. Is it normal to feel more pain after kegel type exercises? It is really scary to me. Thank you!!!

Yes, kegel type exercises will often cause more pain in patients with long term pelvic pain. Kegels will cause more stress in tissues that are already stressed. Pelvic PTs can teach you ways to calm the body instead.

I am wondering as well if PFD can also cause symptoms such as incontinence and painful intestinal bloating. This has been my problem for a few years now but my gastroenterologist cant find anything, so it makes me wonder if its less about the intestines but the muscles that surround the pelvis. Pelvic floor muscles weakness and/or poor motor control and coordination can cause and/or contribute to incontinence. However, painful intestinal bloating is likely due to something other than pelvic floor dysfunction. We do see these symptoms in conjunction with pelvic floor dysfunction, but the source of the bloating tends to be more viscerally related. If your gastroenterologist has been unable to assist, you may consider consulting with a Integrative or Functional Medicine specialist.

I was diagnosed with PFD after a year and 3 months with horrible chronic constipation ( this is the result of 28 years of chronic IBD-D ) doc sent me to physical therapy and quicky medicaid refused to pay so I had to stop, only made it to two sessions... Thanks its a Wonder full video about the pelvic floor dysfunction as recently i get to hear from my urologist where i had changed around 10 urologists within 2.5 years and spent thousands of dollars on antibiotics and diagnosing for every 2 or 5 months where i was not getting any outcome for my prostrate pain problem which usually occur after ejaculation and a type of episode goes on to infection in kidney. Earlier the urologist were treating me for Chronic prostatitis and cancer as the symtoms like bleeding in stools, urgency in urinary other things were same and i was in great depression as i was not getting any solution for the problem but ulitmately i found a great urologist where he explained me the same problem you have discussed as there is no knowledge to 90% of the urologist. Really appreciate the video also if any solution to this problem

I have been having hip pain for the past two years. Occasional burning pain in urethra and frequent urination. Lately I sometimes see very subtle swelling on right side of pelvis after walking a long distance and it dies down. Does not look like a bulge and does not show when coughing or straining. Only after walking. Sometime the pain is not there at all and sometimes it is entirely in my hip flexor area and inner thigh muscles. Seems all related. Thought it might be a hernia but no bulge other than a slight swelling after walking distance and my hip actually hurts at that point. Several months back I was having a weird thing where I blew my nose in the morning and had a sharp pain in flank on same side. That has not happened lately. I have not gone to doctor because these things will not reproduce on command.

There are many things that contribute to libido, and pelvic pain is one of them. It can be hard to want to have sex when you're in pain. If you are able to see a urologist to rule out medical contributors, a pelvic floor physical therapist would be appropriate to address the muscular side of things!

Thank you for this beautiful video! It's like the empathy can be heard while you speak... As somebody who never got the official diagnosis and also have health anxiety, I would like to ask you if this pain around the lower abdomen and pelvic area could be coming from overthinking and fear from this condition? I honestly cannot track what causes the flareup. First I thought it was cold weather, then I thought it was some foods but I am not sure cause the flareups come randomly. Urologists just tell me to chill and don't worry but pain is sometimes there...

Is it true that a urologist would not know who to direct you to if you were a male who needed pelvic floor attention? I would imagine they would have knowledge of this and solid recommendations and tons of experience. Perhaps I was wrong

It depends on your provider and their experience/knowledge of pelvic floor physical therapy. Your urologist might not have a specific person recommendation, but rather refer you to PFPT, and have you find your own provider. We would imagine they are knowledgeable and may know colleagues for specific recommendations. However, we have directories, should you need help finding a provider.

@@PelvicHealth I really like the way you explain things in detail while not making a person feel uncomfortable or cringey. One thing I wish would change, and would have helped me greatly, is if the medical community stopped calling it Erectile Dysfunction. That is also a hurdle men have to swollow before they seek help. That labeling says there's something wrong with me, I am dysfunctional, not good enough, a failure, negative... negative... If I had the power, I would call it EFI Erectile Function Issues. Most men with issues don't have complete erectile dysfunction / total impotence. If this changed, it may well have the effect of bringing more men into the clinic to get looked after with less shame...before there is a major issue. As we all know, men don't handle critique very well, and using negative labels tends to dissuade many men from seeking help.

While it is best to monitor PSA trends over time with your urologist, a number higher than 4 typically indicates infection (prostatitis), underlying prostate cancer or an enlarged prostate (benign prostatic hyperplasia). All of these diagnoses can cause pelvic pain symptoms such as painful/difficult urination, pelvic and lower abdominal pain, and/or urinary urgency. If the above diagnoses have been ruled out, you may have chronic pelvic pain syndrome (CPPS) Type 3A in which case you have no positive bacterial tests, but there are inflammatory markers in your prostate secretions. Urinating should never hurt. Sharp feelings after peeing can be a symptom of pelvic floor dysfunction, and/or pudendal neuralgia and/or prostate inflammation (with or without infection). Many patients with these symptoms have a combination of factors that contribute.

I’m a mad with a tight pelvic floor causing chronic constipation. Most likely caused by clenching my stomach and pelvic floor in a resting state all my life. Doing all I can to breath and relax my pelvic floor now but it’s slow -going. Have you seen any benefit to internal work? To try and massage the muscles out? Or wearing plugs?

I'm sorry to hear that you've been going through this but I'm glad that you are working on breathing and relaxation strategies! We absolutely find manual therapy (both internal and external) as well as working on coordination training with a physical therapist very helpful in addressing tight pelvic floor muscles. I do not recommend my patients wear plugs as a solution to this problem but rectal dilators can sometimes be appropriate. Seeing a PT who is trained in male pelvic pain sounds like it would be helpful for you! Good luck with your pelvic health journey!

For years I have had odd unexplained aches in the region of my body that no one explained, or listened to me and basically blew me off. Several weeks ago after experiencing back pains that felt like a gang of motorcycle thugs had kicked the you know what out of me. followed by high body temperatures with no reason for them. Now, thanks to U Tube I know what the problem is. This is another area where men have little information, or how to get relief.

In these cases, it is possible that the pelvic floor is not the only reason why someone is having their symptoms. There are so many things that contribute to these symptoms, including things like blood pressure. It is also possible that the physical therapy plan needs to be altered or adjusted. It would be best to consult with your providers or get a second opinion.

@PelvicHealth My providers and urology in general have absolutely no idea what causes vascular erectile dysfunction, how to locate the source of poor blood flow and how to fix it. I am on my 4th urologist soon to be 5th. Still looking for a urologist knows how to fix erectile dysfunction. All they do is hand out Cialis like it's candy. They have no idea what the problem is and how to fix it. That is why every guy with erectile dysfunction is desperately seeking answers by doing their own medical research.

Any recommendations of physicians in Dallas DFW who are familiar with this topic and can properly diagnosis me. I am 38 and just started having this soreness in my pelvic region and around my lower back to buttocks area. Ive experienced burning while urinating that comes and goes at times. I also had PE for about a week at random times while just laying around, that has stopped by the pelvic pain is consistent. Any recommendations?

Good evening, thank you for this video. This was extremely helpful and insightful. I know this comments is quite late to the party but I had a question. I am a 26 year old male who has been dealing with pelvic floor dysfunction for years. It resulted from most likely from incorrect weight lifting, incorrect breathing and bracing and a hip labrum repair. I made significant strides in getting better. A little over 2 weeks ago I had inguinal hernia surgery to repair a surgery. Surgery went well and have been recovering. As I’ve been recovering I’ve noticed my pelvic floor has felt very tight and have been constantly having to “let go” of the muscles typically felt in the perineum to try and relax them. With that has come pubic bone pain, post urinary discomfort (slight burn or tingling for a second when urine is left in the tube). Based on the surgery to a area close to the PF is it possible is “guarding” per day and has returned to its tightened state for protection and the trauma that is a surgical procedure? It was a scary thought that something worse could be going on but I’m trying to stay grounded by saying I’ve experienced some of this before. We would say it is possible, given your history, that the inguinal hernia repair has caused some guarding of the pelvic floor muscles as a response to the discomfort of surgery. On the other hand, you should consider that the inguinal hernia repair has compromised the ilioinguinal nerve, specifically if mesh was used to repair the hernia. Common symptoms of ilioinguinal neuralgia are pain along the inguinal canal (groin), pain at the pubic bone, pain at the base of the penis, and sometimes pain in the urethra and/or with urinating. The ilioinguinal nerve is compromised, in varying levels of severity, about 30% of the time during an inguinal hernia repair.

I have had a scrotal lesion for about a year now , I haven’t been tested for sti or std but I’ve been on w2-3 rounds of different antibiotics and nothing , my thigh my lower back and my balls hurt ! @@PelvicHealthI occasionally have a burning sensation while urinating!! I also have a peeing sensation in my perineum area that remains after urination. When I bend over I feel something pulling in the perineum sometimes this is accompanied by pain and as if there is a wound there. When I occasionally pretend to hold my pee I feel a cutting sensation in the perineum!! Sometimes I can't sit down then I feel like something is cutting or a pricking feeling, I think the nerve

@@PelvicHealthI have already been to a physiotherapist and they said that the front of the perineum is tense, debulbospongiosus and ischiocavernosus, but it causes so many complaints that I cannot believe that the pelvic floor alone can cause so many complaints!! I still requested an MRI of the perineum or am I exaggerating? Could there be some tumor in the perinieum? The bladder and prostate have already been examined by the urologist and there are no abnormalities.

Typically your exercises are given after your initial consultation, as the pelvic floor PT needs more information about your pelvic health & pelvic floor, along with the goals you will be working on. During the exam, once the pt leaves the room, you will undress from the waist down & cover with a sheet. As the pt does their exam, you will be covered during it, you will not be naked or need to be erect (as the exam does not actually exam your genital's, but rather your pelvic floor & surrounding area). I was told if you don't get improvement after 6-8 PT sessions, it won't get better. Do you concur? I have been to Pelvic Floor Rehab Medicine and was sorely disappointed.

Great question! Usually patients will start to notice a change in their symptoms by the 6-8 session; however, this can vary with everyone. Also, there is a large spectrum of how much of a change they can experience. For instance, things we will ask a patient is if they notice a change in the frequency or intensity of their symptoms. It’s definitely a process that isn’t linear due to the complexity of pelvic pain. And if there isn’t any response to treatment around 6-8 visits that doesn’t mean we think pelvic floor PT isn’t working it just may mean we have to shift the treatment plan. Pelvic floor dysfunction can include dyssynergia, and pelvic floor dysfunction with and without dyssynergia can lead to inflammation and/or nerve compression that results in post-ejaculatory burning. The pelvic floor muscles rarely act alone, therefore a physical therapy evaluation and PT treatment will address impaired muscles, tissue, nerves, joints, movement patterns, etc in the trunk, pelvis and lower extremities. @@thehardlife5588 There are different mechanisms of action with various pain relievers, this can be one reason. Of course, if there is an actual infection, then antibiotics may help because its fighting the infection and there is pain relieving agents in many of them. Hope this helps!

I’ve been having issues with frequent urination lately that’s seems to have gone now but I’ve been having a feelings of tightness / constriction in the area half way between my belly button and penis. I think this is my bladder area but maybe slightly higher it’s hard to pinpoint. Do you think this could be caused by pelvic floor disfunction. Primary bladder neck obstruction is a functional dysfunction rather than a true obstruction like prolapse, enlarged prostate or scar tissue formation. If the functional obstruction is rooted in a neurological condition, this can be very challenging to address with pelvic floor physical therapy along. If it is rooted in poor coordination and pelvic floor muscle tone, then pelvic floor PT may be very helpful. I’ve had these type of symptoms. I did see a PFD PT. She was great and helped me understand more. It really started last year after a bike fall. Which lead me to not be so mobile, which lead to 2 very bad constipation experiences. Which I think was the catalyst to this. So once I got better I started riding again. But without knowing this wasn’t a good idea. I noticed through the day and getting towards the end of the day a burning sensation between the anus and penis area. Like a sunburn. So after seeing the PT. Had some exercises and stretches. It got better for sure. I decided to try and ride again and it came back. Very frustrating. Now again it lingers and gets worse thought out the day. Mainly when sitting for work. Even though I get up every 30mins. I may go back and visit the PFD PT for a review. I guess start over again. I can run and seems fine. Maybe later I can feel it burn up again. Mentally challenging too… anyway great video🙏🏻

From what you’ve said, it sounds like something caused by pudendal nerve injury. Look into that and then see a pelvic floor specialist/physio and your physician.

@@civic400bhp thanks. Yeah definitely… the “sun burn” feeling has calmed down. Not as annoying like it was. PT got me to do happy baby stretch. Breathing/ relaxing etc. also gentle massage of that area too… it’s slowly getting better. So just gotta be patient. Before my bad constipation experience. Riding the bike etc was no problem. It’s only after that it’s flared up…

Bloating and/or gas is likely due to some more gastrointestinal rather than due to pelvic pain. Increased tone in the anal sphincter muscle may contribute to the occurrence of anal fissures.
