Vinicius Junior set up the Madrid goal for Rodrygo, who tapped in at the second time of asking after a snap save from Ederson.
Kevin De Bruyne deservedly levelled for City in the second half of a one-sided game.
Rudiger came closest to scoring in the first half of extra time when he fired over from close range just before the break.
City are two points clear at the top of the Premier League while Real are eight points clear in La Liga.
