Opening a vertical pipe thats suspected of being under pressure with no adequate PPE for their faces, leaving raw sewage lying in a public alley way for quite a considerable time, opening a manhole in a public alley way and leaving open with no protection or indication such as cones being put out and leaving this manhole both open and unattended whilst the alley way was open to the public, thats all I could see on that first job.
