A bit of context on why Quack Doc isn't all that good at his job , Doctors in the rear palace were usually never any good, after all if you spend your whole life learning a difficult profession like being a doctor which guaranties you a good future, would you really decide to go work in a place where mistakes could cost you your life, not to mention you'd still have to be castrated just to work there like all the other men, doctors are not exempt from becoming eunichs.

The sleep deprivation to my understanding is referring to the fact that she was basically attending to him every night while also needing to raise her new born daughter which is by itself a job and a half. With the lady of the crystal palace healthy again (who I believe is his actual #1) he went right back to her freeing up time to sleep for Gyokuyou. I'm actually 50/50 on if she is actually just happy for the break and extra rest or if she's doing that to make maomao uncomfortable due to her technically helping her rival as one of her lady in waiting.

It's easy, from our modern perspective, to assume that Maomao's skills would give her some amount of protection or privilege, but it's really important to understand that *they really, really won't*. Concubines, especially the highest ranking ones, are always going to be high ranking nobles, and even their ladies in waiting are going to be some level of lower nobility, most times. For women, those ARE some of the highest positions they can have and aspire to in that society. For the owns who don't have any position by blood and are simply commoners, being normal servants like Maomao's friend is mostly where it's at, and blood/family simply ranks higher. Regardless of her skills, Maomao is the lowest rank in that palace. The ladies in waiting who are noble by blood outrank her by virtue of their birth which is held above personal skill, neveind that being a woman already puts her in a dangerous position. Maomao is useful enough to be asked for help, but by the standards of that society, and in the palace of all things, she's ultimately disposable compared to anyone ranking over her, nevermind the polytical implications of siding with a common born servant girl over the daughter of a noble. Sure, the emperor WOULD be able and allowed to make that choice, but the implications towards a noble family to be slighted like that could cause tensions and make a polytical mess. Maomao often thinks about how disposable she is, and while to us as the audience, and to some of the characters close to her, that clearly isn't the case, the fact of the matter is that by every standard of her position and society, she is considered that way, and that's something she has to live with. You aren't going to ignore the emperor's orders and just let someone die when, by those standards, regardless of your skills, you are the most disposable person in the room, and the one most likely to suffer any consequences.

As for why the eunuch got such a severe punishment, but the lady-in-waiting was just put in confinement in the anime, generally the ladies-in-waiting of high-ranking concubines would be chosen from lesser noble families that served the concubines family. So, they're too high of rank to be beaten themselves. It's also probably why they were so incompetent with the care for a sick person. It's not something they ever bothered to learn.

I would have the same reaction to. Because I know my taste in men is aesthetically the opposite of Jinshi. Many women prefer soft, disciplined and empathetic personalities in large muscular bodies...
