Well to me, whilst I agree to some people who did their own interpretations of the backstory of Super Mario Dolor, I personally believe that Luigi had died before his brother's very eyes in order to protect Mario from Bowser or some other baddie on one of their adventures. The reason I believe this is because, before you go to the final castle and take on the giant Shadow Mario - or Demon Luigi if you collected all five green stars and found the secret room to get the best ending in the game - you end up fighting Bowser. Which in turn, was the reason why Mario felt so guilty about Luigi's death; Because he felt he could've and should've done something, _ANYTHING_ really, to stop Luigi's sacrifice.

I think the story is that Luigi probably died on one of Mario's adventures. Because of this Mario then gets nightmares about one of his past adventures which probably was the one where Luigi died. Also this probably means that Mario has depression because of his brother's death, which is why he uses pills in the normal ending. But maybe Mario also used the pills to forget about Luigi and the day of his death. Meanwhile in the best ending, instead of using pills Mario faces the fact that he will never have Luigi again in his life, so he brings flowers to Luigi's grave. Also I think Peach and everyone else aren't in the game because maybe ever since Luigi's death Mario wasn't that sociable and he probably wanted to be left alone.

Heres my perspective: In the normal ending, Mario is getting droven insane about Luigi's death, and he just wants to see his brother. I think the pills are his way of seeing his brother again, (death). in the Best ending, Mario decides that it is better to honour Luigi and not to forget but to remember.

The best ending: after the long sorrow of Mario’s best bud death, it was only a dream but the person he loved most could be the one behind all of it. Remembering nothing was real about it of his brother becoming an entity of chaos he still wonders. Was Luigi a part of what was happening to Mario in his dreams? Or was it something else? He ponders for a bit but gets a feeling he knows what is really happening. Killing Luigi might be a part of something much more powerful, the thing controlling his dreams but. True evil never sleeps...
