Here is a bit of insight into the characters throwing wide of the base. In Sluggers, the throws to the base are programmed to go to the base and NOT to the character. Bad chemistry throws are inaccurate, and can cause the throw to miss.

Actually, that bad throw was due to bad chemistry. DK and the Kritters have bad chemistry. Sometimes, the baseman will throw off from the other baseman. I feel like, with how much you've played this game, you should know that by now.
