When you’re operating from a position of “the only way anyone would want this procedure done is if they’re clinically insane”, it looks tremendously irresponsible if a doctor agrees to it regardless of informed consent. To people like Matt Walsh, asking for an orchidctomy as a trans woman is essentially the same as asking for a medically unnecessary leg amputation; anyone who would ask for it cannot be trusted to make their own medical decisions and anyone who agrees to provide it should be charged with malpractice. Note; This is not my view. I’m trans myself. But this is how the right thinks.

a social worker is not a licensed health care provider. That's like saying your student counselor is a licensed health care provider. All they can do is write letters on behalf of their client saying 'doctor, i really think this person should be tested for X, as they displayed the symptoms of X when I saw them on (date).' That is LITERALLY all they can do. They don't order a doctor to do procedures on their client. They can't even officially give a medical diagnosis. They don't even have a psychology degree (although they can study for one which will make their work more effective).

I live in Canada. I had monstruous tits that didnt fit with the rest of my body. I was 13 and I talked about cutting them off. I was filled with anxiety, I hid my body, I was miserable, it even made me question my gender. I didnt have pain in my neck or back, so I was worried I was stuck with them forever. It didnt bring in suicidal thoughts, but i didnt feel like I was this body. 10 years later, I had breast reduction (for free bc Canadian health care common w) and I felt extremely better in my body, I even started feeling more feminine. I didnt have to have pain in my neck or back to be qualified to have it for free. When people started saying Gender affirming care is the same thing as cosmetic procedure for cis people, I absolutely agreed, never questioned it.

I live in Canada too, and although my breasts/butt are relatively small (my butt slightly less so 😞) they give me anxiety and I am also going through the process of waiting till I turn old enough to remove them. Thanks for sharing your story, it’s always nice to hear people’s unique perspectives ❤

"This person reported experiencing certain symptoms to their healthcare provider and expressed specific health goals and desires to that provider, and the provider LISTENED, OFFERED INFORMATION, and was WILLING TO PROVIDE CARE! GASP!" If they wanted to spar with a doctor who was keen on sussing out liars, exaggerators, or malingerers, they should've tried asking for opioids or Adderall instead, or should've pretended to be a fat woman with just any health complaint.

This surgery is a carefully-chosen example; orchiectomies are outpatient procedures that cis men can have performed essentially on demand, as long as it isn't for transgender reasons.

For me the reason for detransition was simply that I did not feel like the results of bottom surgery would alleviate my genital dysphoria (the primary driver of my gender dysphoria) and that's a choice *I made for myself* because I weighed the risks of surgery with the pros of it. All surgery has risks of complications and I think it's important that people stop associating the term "cosmetic" with "not necessary for the wellbeing of the patient". Transition is complicated and it sucks so much seeing these conservatives deliberately refuse to engage with the nuance of the subject in order to continue hating it

@Belisarius thank you. I am doing well now and have found ways to relate to my body in positive ways. Understanding my autism diagnosis, identifying as transmasculine and having a partner that understands my relationship to my body really helped my particular case. It's really wild that any detransitioner would assume their experience with gender dysphoria is universal for all trans people

You can say you're depressed whenever you want but medical diagnoses and many prescription meds should absolutely be gatekept. That's literally the function of a diagnosis. If your diagnostic category doesn't include some and exclude others according to specific criteria, then you have a useless diagnostic category. Just look at the fiasco during COVID where people with Lupus and other serious autoimmune conditions couldn't get their hydroxychloroquine because of widespread misuse of the drug for covid treatment. Even people with medical expertise misdiagnose people all the time and it's infinitely worse when medically ignorant laypeople people try and diagnose themselves. It's not gatekeeping that's the problem. The transphobia and the profit-run medical care is the problem. To be clear, I think transgender people, like everyone, should have free medical care. If their medical providers, using the best available evidence, think that entails puberty blockers, or HRT, or surgeries or whatever, then they should get it.

More than that. It's literally saying you don't have any right to any medical care unless you want to "prove it" EVERY TIME. Like imagine telling your doctor "hey man I think I have the flu, I have all the symptoms I've had previously with it so what medicine can I buy for it again?" And them denying you that information because you haven't been tested enough to find the specific strain. Like in this clip the guy was lying his ass off to get a surgery he didn't want to prove something to twitter, are doctors just supposed to assume everyone is doing that all the time?? And are they supposed to assume responsibility for that behavior instead of the adult whose doing that?? This shit is why it takes an average of 10 years to get diagnosed with endometriosis, people trying to make it harder for others to get sex-related treatments "incase they're lying/confused". This is straight up denying people medical autonomy. You have no right to what happens to your body unless you can "throughly prove" you need it/want it to absolutely everyone with that mentality. Edit: and that can lead them to saying you need to "throughly prove" you don't as well, see the 10 year old abortion case.

That sucks, I've been denied healthcare that I desperately needed & almost died, & another time was left in so much agonizing pain bc of a negligent Dr not giving the care I deserved that I tried to unalienable myself. I wouldn't wish that on anyone & will adamantly argue for All Americans to get the care they need & deserve whatever their health care needs- whether physical, mental, or cosmetic (i.e. burn & accident victims, & ppl with conditions where the classication as such belies a genuine need for reasons of both health & to erase the stigma these ppl live with, in their career, in society, & difficulties with self image & relationships & mental health- such as need for dental implants or dentures, treatment of scars acne & other skin conditions that all or many insurance doesn't cover, etc. I do think there does need to be some medically necessary "gates" so to speak, to protect ppl who have severe depression & suicidal ideation that is NOT a result of the dysphoria, bc a painful & major surgery & recuperation period in which they'll rely on potentially addictive painkillers & could develop a psychological dependency afterwards, also there ate some types of mental disorders where ppl actually want to remove body parts that don't need to be, for reasons like real or phantom pain, or other psychological troubles giving them the urge to change their body, & these do need to be carefully screened for. Also, it's absolutely not OK, & it is an example of the same conservative patriarchy that maligns trans folks being harmful to cis women, proving the 2 groups share a common oppressor, but it's actually EXTREMELY Common for adult cis women to be denied to ability to chose to change HER body to permanently remove her chance of pregnancy. Women eking to have their tunes tied are almost ALWAYS denied until at least 40 yo in the U.S. it doesn't matter if she is ADAMANT she never wants kids or if she already has 2 or 3 and doesn't want more.... finding an Obgyn to perform the operation is almost ALWAYS impossible, without a medical condition that makes pregnancy extremely dangerous to her. So obviously trans folks deserve care, just an example to show that many other groups experience terrible treatment from Doctors and from the medical system ad are infanticide when they say that they know best what they want for their body.

My doctor literally had to lie and make up certain symptoms to my insurance company for better diabetic supplies which has brought my a1c from the 9s to the 6s. It's an insurance problem not a trans problem.

It's not the same though for one key reason: You don't see a short guy, immediately start call him mam, and struggle to treat him like a man even after he corrects you. Gender affirming care literally changes the way a person interfaces with the world. The societal difference between how short men are treated and how tall men are treated is not remotely comparable to how men vs women are treated. Not to mention the current political climate. I get that it sucks to be short in a world where being tall is rewarded, and it can absolutely decimate some guys' confidence, but if I passed better, it might literally be the difference between being a martyr or not for just going to the grocery store.

i had to get two separate letters from two different mental health care providers to have this surgery. The first was from my therapist, the second was I had to find a different therapist and have a conversation with them to get another letter. This was kinda like that second conversation. Is there any other medical procedure that requires this level of scrutiny and gatekeeping? The therapist knew it was a waste of time that was simply required as an ass-cover for insurance but still went through the process of asking me questions for an hour.

My hysterectomy took 8 doctors over 5 years before a doctor agreed to it because as a woman, it is assumed I am straight and my husband owns me and I need his permission to get the surgery, and I am a dumb woman who can’t make decisions for herself and I’ll regret getting a surgery to remove an organ that was ruining my life. I needed two letters from a psychiatrist and a therapist, and even in the end, the doctor refused to accept my reason for getting it done. The surgeon begrudgingly performed the surgery. It was miserable. Trans women and men and cis women’s healthcare is so f-ed up. Doctors really only look at patients through a cis white man lens.

When an insanely authoritarian conservative like Walsh thinks of healthcare it's important to understand that they think the whole medical system should basically tell you what you can and cannot do. There is no presumption of bodily autonomy, no expectation of freedom. You get what you're given and do as you're told.

I'm a guy who had pretty severe gynecomastia. Had a single tit that was just chillin on my chest. You'll notice I say had, because I had it removed with no questions about why I wanted it gone or any kind of extra steps. The only thing that was a small roadblock was the waitlist because I managed to get it through the NHS. There's no reason why trans people shouldn't get the same level of treatment and respect for their decisions

@@DangerJim-vq5uh they usually do screen for mental health conditions that effect your ability to make an informed decision, but having a history of suicidal ideation doesn't actually make you unable to consent. The regret rate for trans surgeries are very low and I haven't seen any evidence that suggests that barring suicidal people from transitioning has any benefits. Generally speaking people's mental health improves after transition, so it would be cruel to prevent mentally unwell people from accessing healthcare that can improve their life.

As someone who's been through trans surgeries as an adult: the reason she keeps calling the letter a "persuasive essay" is not to convince any doctors to give you your surgery, nor is it to convince the patient that they're trans, as I think Mald Walsh is trying to imply. It's just to convince your insurance, because most US insurance companies will do whatever they can to not cover your medical needs. To doctors (at least the ones I've seen, and I'm in the middle of the Bible Belt) the letter is just a check on the list of rules they have to follow to keep their medical license. Your psychologist/therapist knows what the letter is for, they know how to diagnose you, and they're the ones who actually care about you getting the healthcare you need. That's why they're emphasizing how to do it in a way that actually works to get insurance coverage. It's because they know how many hoops we need to jump through and they actually care about helping.

There’s definitely dynamics of hegemonic power that you aren’t really acknowledging. Shortness is usually a disadvantage of course but short people have never been categorically oppressed by virtue of their shortness. I had gynecomastia but only in my right boob since I hit puberty. In my mid 20s it got so bad that I actually subconsciously changed my posture to accommodate for my lop-sided chest size. It didn't hurt me but I was extremely self-conscious about it. Finally saw several doctors and they kept telling me it was cosmetic to get it done and insurance wouldn't cover it until I finally found a doctor who asked if it caused me pain and then proceeded to define what she considered "pain". Once that diagnosis was on record I was able to get coverage to get a mastectomy!

It’s sucks so bad trying to convince insurance companies to cover anything. I work in pain management and at the beginning of the year, we had to stop giving treatments to patients that said their pain was under a 5. You know, because maybe what we were doing was helping. They refused to cover it. We’ve been making sure patients tell us what their worst pain was throughout the month, that way, we can keep treating them. We works side by side with an ortho practice too and we’ve seen insurance deny hip and knee replacements… they deny MRI’s and x-rays we need to see what’s wrong and how to treat the patient and say they aren’t “medically necessary.” All to save themselves some money. If they could have their way, they would go back to not covering people with pre existings because sick and hurt people don’t make them money.

The greatest one is effing viagra. It's a HEART MEDICATION. The SIDE EFFECT is erections. We literally prescribe it more for the side effect than for its original, main effect. (I forget what it does exactly but it's for heart problems) Also we use it for some animals for I think blood pressure, as an emergency, IIRC. (human meds having weird effect on animals is a really good example of using the same compound for multiple things)

It takes several months to get approved for an Orchiectomy. I know this for fact. It took me several months of questions with a therapist before I was given a letter of approval. I'm also Intersex and need the surgery but still had to go through a lengthy process. It doesn't matter whether you are an adult or not you can't just ask for Orchiectomy and receive one. It does not work like that in the States.

Simply living in an unsupportive environment where you don't have anyone to help you after surgery will keep you from getting approved. For example, most of my friends live in Southern VA and I live in the DC area. I almost didn't get approved because I didn't have two people to provide to the therapist to prove that I will be cared for post-surgery.I also have to show I am currently mentally stable. If not, I have to undergo psychological treatment and be cleared before being approved for the surgery.. The process is complex. More complex than I described.

Gender dysphoria =/= body dysmorphia. Us CIS people experience body dysmorphia. Trans people experience gender dysphoria. It's much harder to them, cis people can get over it with a minimal ammount of therapy and not even surgery. Trans people HAVE to have gender affirming care.

I mean, I think there's a fair argument that cis people do sometimes get treatments that are deemed medically necessary for gender affirmation purposes. I don't think it's unique to trans people, even if we disregard cases of body dismorphia. Gynocomastia removal is often seen as medically necessary in young men, but the main reason those surgeries are performed is because of how having excess breast tissue can negatively impact body image. Its very common for cis women to get implants after breast removal for cancer and this is typically done for mental health and body image reasons. Trans healthcare is obviously important, but I don't think it's unique.

I agree, its not the SAME, but you have to acknowledge that body dysmorphia can he horrible to, I wouldnt say that "one is harder than the other" at all, both things can be extremely traumatizing and horrible. Also, some trans people dont have any or very small amounts of gender dysphoria and some do actually find out how to allieviate it for themselves to a point where they can live a fulfilled life (Even tho this isnt the majority of trans people) Btw, some cis people do experience gender dysphoria too! But seriously, its so hard to get gender affirming care, not even just because of doctors and those unnessecarily long ways and "requirements" that you have to go through, but also due to how much misinformation exists in regards to this topic. Coming from a transperson btw (Sorry if I made any spelling mistakes or something, Im 14 years old and still learning english)

Walsh is presenting these posts and videos and he's really stretching for the sinister connotation. Of course you write a "persuasive essay" to explain your position and why you need treatment of whatever kind. That is literally what such a writing is called. Does Walsh prefer a legal brief?

It is sinister. Like if you needed triple bypass surgery it's because your arteries are blocked and you'll die without it. And if you had to game the insurance to achieve it then so be it which is really fucked up but I digress. But these are NOT necessary procedures they're elective and cosmetic and honestly these people are teaching others how to FAKE a diagnosis in order to get invasive procedures done. THAT is sinister.

Neither do you need a letter from a social worker to go have your own elective cosmetic surgery even if you’re obviously addicted and at a surgeon for the 26th time to fix the botched surgery from the 25th.

Trans surgery is more or less reconstructive surgery. Gender dysphoria is the medical condition that's being treated, and it causes a similar amount of distress that a cancer patient might have after losing their breasts. Both would need reconstructive surgery to make them feel less gender dysphoria.

Here's the thing, I'm a transhumanist. I believe there SHOULD be VERY LITTLE regulation on what people are allowed to modify about their bodies. I LIKE these surgeries being accessible.

Right? I personally dislike excessive piercings and tattoos. As in, I would never get them myself. But I'm not all over the Internet trying to convince people that anyone with that aesthetic is subhuman, etc. People are people. We got one life, and unless it's actively harming someone (keyword here: actively) we should be able to live it in whatever way we like.

On the leg lengthening surgery thing: there are also people with dwarfism who have extremely bowed legs that make it difficult to walk. By letting people without dwarfism to get the surgery, you put more resources into the research of the surgeries and make it safer for the people who would greatly benefit from it.

Yeah, that really sucks. The therapy center that I got my Vyvanse through was really hard to get in contact with and needed me to call in every month for a refill (which I understand because it's a controlled substance, but that's not very acsessible for people like me who has a very extensive and documented problem of phone anxiety and no transport). I do know that there are multiple services where you can get your RX at a discount if money is a problem for you.

I use to take 100mg of spironolactone per day as part of my gender transition. My mother's boyfriend is on 20mg per day due to age and prostate issues. She made me promise never to let him know they we were both on the same medication, because if he knew what I was taking and why, he might stop taking his

I would like to point out that their efforts to keep minors from getting puberty blockers effects non-trans kids too. My sister might have to take my nephew out of the state to get him puberty blockers so he has a chance of not being a Little Person like she is. She hit puberty way early, and we had no idea it would cause her to be so short. She's fine, but she needs a cushion to drive and stuff, and she's had to deal with people not taking her seriously and trying to mess with her her whole life. (She had a friend in college who dumped water on her EVERY DAY their first year as a joke. You know, "to see if she'll grow." Not the first time she'd encountered that joke, btw.) She's well aware of what it will mean socially for her son, and that it will be worse for him as a man, which is why it's so important to her. (Short kings are kings, neither of our parents are tall and I'm fairly short myself, our bro will also never come close to 6' unless he suddenly develops a tendency to lie, and we're all fine but there's a lot of shallow people who don't realize that and will give him heck for it. Also, the world is built for people of a certain height, and I'm guessing she doesn't want him to have to rely on strangers or climb the shelves when grocery shopping for the rest of his life.) In his case the cause is apparently linked to the medication to treat his severe ADHD, apparently it can do that and maybe no one warned them or they were just too desperate. He's actually on growth hormones now to counter it, but they'll only work until a certain point in puberty and he's running out of time. So yeah, way to go you political f*****s in Nebraska! I hope you frick yourselves with the business end of a rake!

Hormone blockers are used on underage kids who started puberty too early. But yet if it's trans kids, they want to cry "It's dangerous" Hormones are used on people of all ages to help with imbalances. My childhood friend used hormones from age 9-10 because he was underdeveloped for his age. He was shorter than everyone else and late on hitting puberty as he got older. The hormones helped correct this. Yet if trans people need hormones "It's dangerous"

I had a breat reduction at 17 because it made my back pains so bad and i was genuinely so uncomfortable. I wore hoodies no matter the temperature..... In texas. While it was technically considered cosmetic, it was necessary and 1000% made my life better in every way. The only reason they were slightly hesitant on allowing me to get the surgery was because there was no guarantee that they wouldn't keep growing.

Thanks for saying that we shouldn't make fun of people who get cosmetic surgery, even when it's "Dumb" People who do this, don't hurt anyone, and it is a sad thing that people feel such a discontent with their body, such a distress and pain, that they feel they need such surgeries. Of course I would advice against such surgeries before hand, however, it is simply not our place to judge people for the decisions they make, as long they don't hurt people. And frankly, I don't feel the way, people feel who undergo such surgeries, - but that's the point. I can say, what they are feeling, but I understand, it is complicated and hard for them. And sure it seems unrationell , ( and probably is) but that shows who much pain it causes. we should criticizes the system (or whatever caused them to feel that way,) not people who suffer,

Right now at this very moment there are veterans being advised in a similar fashion to what is seen here in order to obtain care from the VA or benefits. This kind of coaching is ubiquitous with in our systems and is not considered wrong at all. So this is a big nothing burger.

As intersex who had to undergo pretty severe surgery at birth to even live and then was on HRT around puberty, I can say that "gender affirming care" is pretty freaking important thing to exist and ideally be covered by insurance.

it's not like they chop the balls off within 20 minutes- you still need a 2nd letter usually, you need to have been on hrt for a year usually, and you also still have to actually have the consultation and schedule/wait for surgery date.

This is quite literally saying you don't have any right to any medical care unless you want to "prove it" EVERY TIME. Like imagine telling your doctor "hey man I think I have the flu, I have all the symptoms I've had previously with it so what medicine can I buy for it again?" And them denying you that information because you haven't been tested enough to find the specific strain. In this clip the guy was lying his ass off to get a surgery he didn't want to prove something to twitter, are doctors just supposed to assume everyone is doing that all the time?? And are they supposed to assume responsibility for that behavior instead of the adult whose doing that?? This shit is why it takes an average of 10 years to get diagnosed with endometriosis, people trying to make it harder for others to get sex-related treatments "incase they're lying/confused". This is straight up denying people medical autonomy. You have no right to what happens to your body unless you can "throughly prove" you need it/want it to absolutely everyone with that mentality. And that can lead them to saying you need to "throughly prove" you don't as well, see the 10 year old abortion case.

Tbh it kinda fucks with me when other trans people are like ‘oh our surgeries are NECESSARY and GOOD and NOT COSMETIC unlike those cis people who are just fucking around for the hell of it they don’t NEED these things like WE do’ cause like. I’m nonbinary, and I want top surgery. In my case it would absolutely be cosmetic- I’m not in distress about my chest I just think I would like it better if I got top surgery. There isn’t a clear line between trans surgeries which are always life saving and necessary and cis surgeries which are cosmetic and stupid or whatever, we’re all on a spectrum and we’re all looking for the medical care which will help us feel the best.

ALSO that’s the kind of thing this social worker is talking about- if I eventually get a consult, I fully expect whoever writes my recommendation to play up the severity of my dysphoria to get it covered by insurance, because if they said ‘oh they just kinda want to do it’ then it wouldn’t work.

Just to add my two cents, as a trans person, gender dysphoria for trans people is generally not from societal standards regarding appearance. It is something internal that we deal with, likely due to some brain development that happened in the womb, wherein certain aspects of our body cause us extreme distress. We don't get gender affirming care to fit in, but to alleviate this distress.

I agree with this so much. I understand why people are trying to make this argument but it's so frustrating. Insecurity and dysphoria are not the same thing. Saying "it's exactly the same" is just not true. Cis men don't get leg lengthening because they have an entire lifetime of being told they are a woman/girl and are doing something to feel more like a man. Trans people don't start from the same point. We have a lot of complex reasons for getting surgery, and not being the stereotypical attractiveness and being insecure is very different than crippling dysphoria.

Nikki Haley said that mastectomies are irreversible which is hilariously wrong. Women have been getting double mastectomies, during the treatment of breast cancer and to prevent cancer in some high risk people, then getting breast reconstruction surgery for decades. I guess you wouldn't be able to breast feed but that doesn't seem like a huge problem. Why isn't it enough to say unneeded surgery is bad and we should try to avoid them in all cases.

This debate is interesting to me as a European. Here the NHS does cover cosmetic care such as gender affirming care or boob jobs. It's understood here that people's perceptions of themselves can affect their mental health in a big way so a lot of cosmetic surgery or care is deemed the same as needed healthcare and is covered

@@EmpireGamingWynter I can't count the number of drag acts I grew up with there were so many, and some are still mainstream like Mrs Brown's Boys (although I can't pretend I'm a fan of that but only because of the quality). From a young age I was introduced to Eddie Izzard, nothing 'sexual' about their act whatsoever, the most age inappropriate thing they did was say 'fuck'. Obviously Eddie (Suzy) is trans not drag but it did open my eyes to the world of cross dressing and gender fluidity. Grew up watching drag performances in League of Gentlemen, Mighty Boosh, just about every single British sketch show, Little Britain, Blackadder, loads more. Then there is panto of course, which happens in multiple towns and cities every single year and was never attacked by the right-wing over here. I'm getting sick of US culture war crap being forced on us! It's nothing to do with us, they should leave it over there where it belongs, or better still get rid of it entirely and stop pretending they aren't just looking for excuses to be homophobic.

You have to have a psych eval to qualify for weight loss surgery, too, to make sure you're capable of dealing emotionally with a the impacts. Insurance won't cover it without it.

L take at 12:10 Short guys aren't getting clocked as women for being short, they are being clocked as short men. People don't see a short man and immediately assume they are a woman. If they did it would be the same thing because that would destroy their desire to engage with the world--like transwomen without FFS--but again that's not the case.

People with excessively large breast will sometimes get them reduced to make it more comfortable on their body. Thats a cosmetic surgery that has a direct affect on their mental health and well-being.

The difference between men getting leg lengthening surgery and trans women getting facial feminization surgery is that these cis men had a male puberty, and their height is a part of their maleness, whereas, for trans women, their face is only that way because they went through the wrong puberty. For the cis men, they were supposed to be short; it was how they developed as men. For trans women, their faces weren't supposed to be masculinized.
