1. Near misses don't need to be disclosed 2. Ask the patient if she would like her son to know information regarding her condition. If so he can say it might be associated with the procedure his mom recently had and have the surgeon call the patient to discuss it further.

Thanks again for your answer. Q 1 is absolutely correct. Q 2- yes you are right, we need to take permission from the patient to inform her son, but here the main concern is the physician should not say anything without discussing the issue with the previous treating physician. There might be particular reason of giving less fluid during the procedure. The response could be' I will discuss this with DR. X and let you know" or something like this.
