90% of the time it doesnt run and if it does it only shows 1 distance once but ive copyed your code exactly any help?

I suggest double-checking the connections and ensuring that the GPIO pins in the code match the ones being used. If that doesn't fix it, try recreating the 3.3V voltage connection with new set of resistor or use Bi-Directional Logic Level Converter .

How far can it measure accurately?

According to the datasheet it can measure unto 400 cm within a 30 degree cone. So it depends on the size of the object as well. I personally haven't used it for measuring distance more than 200 cm. In my case, one the distance eyes more than 70/80 cm I have seen some bad data. cdn.sparkfun.com/datasheets/Sensors/Proximity/HCSR04.pdf
