It’s exactly what I was thinking as I began watching this video. Edit: The incident I'm referring to occurred at a facility in Reston, VA. Not Marburg. Relax everyone I read a book called 'The Hot Zone' about the German outbreak about 20 years ago. From what I remember the issue in Marburg was that test monkeys were dying in the centre but they were in separate cages on different floors. The implication was that the virus had gone airborne (and was spreading via air ducts) but this variant was *not* communicable to humans... From what you're saying here, I presume this is either a complete fiction or my memory is at fault? Has anyone else read the book?

Adenovirus vector vaccines are not the same as traditional whole (killed) virus vaccines, and have an inherently higher risk of causing blood clots if they get into the bloodstream (typical of about 2% of all injections, according to steroid users). However, if there is a high risk of being infected by the Marburg virus, a 0.1% serious adverse event rate would likely be acceptable to avoid falling into that 20-88% fatality rate statistic from the infection itself. The key variables are the actual SAE rate (currently unknown but appear low enough to allow phase 2 and 3 trials), and the risk of getting the actual Marburg virus. If the latter chance is tiny (the normal situation in most countries outside of Africa), then vaccinations would be unethical. If the chance of being infected (or at least exposed) is 5% or higher (e.g. in endemic regions), then vaccinations would be a great idea - as long as there is full informed consent of course.

After needle stuck into body, the plunger should be pulled backwards first to be sure the needle is NOT in a vein or capillary. Easy way to get the vaccine into the muscle rather than into blood supply, and prevent the clotting problem. In USA where I live it is practically impossible to convince the medical person giving an injection to pull plunger backwards to prevent injection into blood supply. Lots of ways such a virus can be transmitted besides just sex. Just like SARS CoV-2 or Ebola, maybe via aerosolized droplets getting into an unhealed scratch or someone's mouth or being inhaled, or an actual scratch or bite from one of the monkeys, contamination by aerosolized droplets from monkey poop containing particles. In Central Africa, monkeys are a common source of meat; they are caught in the wild, then taken to town markets for food.

Ebola is spread primarily by people washing the bodies of relatives who died from it. That is a cultural thing in that part of Africa. Stop washing dead relatives, and the spread goes way down. Monkey handlers probably got Marburg because monkeys fling poop, and the workers were not wearing effective PPE. We usually don't have much transfer of body fluids from sick people in the US, so while a traveler might bring it a pandemic is very unlikely.

As this is an RNA virus, several questions arise. 1) how fast does this virus mutate? RNA viruses typically mutate fast. 2) How durable is the immunity provided by the vaccine? Typically RNA virus vaccines durability is quite short - circa 1 year. For SARS-CoV-2 it is about 154 days +/- ~45 days for reinfection, and about 7-9 months against severe disease.

To be clear - for most RNA viruses two modes of immune escape occur. 1) first, our immune system intentionally forgets. The energetic burden of maintaining immunity against a rapidly evolving virus makes it energetically advantageous for the immune system to forget the virus in about 4-24 months. Often people believe wrongly that immunity is durable, when it isn't. The mechanism for how our immune system differentiates whether to maintain immunity, or to allow it to fade quickly is as yet unknown. 2) separate from this, the rapid mutation, hybridisation or reassortment of many viruses, especially RNA viruses causes rapid immune evasion. This operates independently from 1), and likely is why the body chooses to forger - resulting in non-durable immunity. However, this high rate of change and immune evasion further shorten the effective time that vaccines can be effective. These taken together are why re-immunization three times a year or more is needed to maintain actual immunity to SARS-CoV-2, and why that needs to be with vaccines updated to the latest dominant strains at that same pace.

When infected by the virus, this is the starting gun for two parallel processes. 1) The incubation period of Polio is usually 7-10 days, during which time the host is not infectious and has no symptoms. 2) The adaptive arm of the immune system takes time to be fully developed, but if vaccinated, this period is shorter than the incubation period of the virus. Hence immune system wins the race and the virus is cleared without the host having symptoms. This reduces the incidence of serious disease and death. Hence the CFR is reduced in populations that have high rates if vaccination.

No, because the smallpox vaccine also works on monkeypox so we already have a vaccine. Even more up until the 70s everyone was vaccinated for smallpox so older people already have some immunity. Military people were vaccinated for it.
