Medication Reconciliation at SickKids

We're serious about medication safety at SickKids. Medication Reconciliation is a formal process whereby health-care providers work together with patients and families to ensure that accurate and complete medication information is consistently communicated. Reconciliation is a different way of saying “matching and comparing” and is important during care transitions such as when patients are admitted to the hospital, when they are transferred from one area of hospital to another, or when they are discharged home.
This year we aim to improve how often we complete Medication Reconciliation during the admission process from 83 to 85 per cent.
