Dr. Liedtke, I wanted to reply because I had Persistent Idiopathic Facial Pain, it was debilitating and persisted for several months. Probably, but not necessarily unrelated, I was diagnosed with a pituitary tumor and prescribed Dostinex (Cabergoline) approximately 5 months after the PIFP started. Almost immediately the pain went away. My pituitary tumor also was treated successfully. Since then I have had several episodes where it felt like the PIFP was returning but each time I have taken Dostinex and it goes away again. I think it is now permanently resolved. I wanted to share this because there are not good treatment options and I think studying Dostinex as a treatment would be worthwhile.

Dostinex is a dopamine agonist and I have read a study linking PIFP to problems with the dopamine 2 receptors, so I think there is some biological plausibility for this and not mere coincidence. Gabapentin resolved my issue on the first dose, the second dose it barely dented the pain, it was completely ineffective after that. Other medications were also ineffective. TN and PIFP react differently to these medications I believe.

Very interesting, Dr liedtke spoke with my wife, Emily, around covids very harsh effects on the trigeminal nerve. Still searching for treatment in Australia but it is slowly becoming mainstream to connect covid and facial nerve pain.

Can you take Clonazapam and Lorazapan at the same time?

I had severe double side of trigeminal neuralgia that was successfully treated with trileptal and fiorinal w codeine for any sudden break thru pain. After many years I was found to have pernicious anemia and went on B-12 injections and the side effect was the trigeminal neuralgia all but disappeared. I will occasionally have a break thru but I feel like the b-12 saved me from the horror of this awful condition. Any doctor who does not offer pain meds for this condition should not be treating it. It feels like a screwdriver driven thru the top of your head, thru your eyeball, and out behind your ear. Just when you think you can't take any more pain, it feels like someone then turned the screwdriver even deeper. It is so painful, you can't cry, you can't talk and you can't function. The very last thing I would want to take is a benzodiazepine.
