Love the video - great overview. One point of contention: while it had been generally believed that fibrosis is a permanent state, there is an abundance of clinical data over the last 10 years that fibrosis can regress. It has been shown with liver fibrosis (hepatitis B and hepatitis C) and also with the kidneys where established glomerulosclerosis is reversible. The clinical data is not yet there with the lungs, however a consistent aspect of fibrosis regression is that when the reason for the cause of inflammation and fibrosis is removed (hepatitis C) or suppressed completely (hepatitis B) that the plastic nature of organ fibrosis that can regress in certain situations. Science changes and is never constant (nor absolute!)

@@MedicosisPerfectionalis Thanks again for the great video; a great review article is from NEJM (Rockey DC, et al. NEJM, 215; 372: 1138-49). This article reviews different pathways & processes for development of fibrosis in various organs along with different treatments. Currently, the most impressive data is with regression of liver fibrosis - perhaps because of the regenerative properties of the liver (Marcellin P, et al. Lancet. 2013; 381:468-75). This showed that (using paired liver biopsies) in those with chronic hepatitis B, compete suppression of HBV replication showed marked regression of fibrosis and reversibility of cirrhosis. At this time period, advanced fibrosis was largely considered to be irreversible. In the 5 year follow-up time period 71 of 96 individuals (74%) experienced reversal of cirrhosis. The data with heart, lungs, kidneys is still emerging - but much research is moving forward based upon a better understanding of the pathways that cause fibrosis to develop. The lungs are very different in that the fibrosis here is unlikely to be as amenable than liver fibrosis. But nevertheless, science moves on and that is what is very exciting

Careful documentation of the past medical history is important in the initial assessment because the cause of the illness is often recognized from the patient's medical history.
