Medusa has a low HP pool (~350 hp). She's typically tanky because of "Effective" HP (eHP) since her mana pool is effectively also an HP pool. However, Axe dunk at level 2 is 375 pure damage (so not reduced by armor or magic resistance) and also Axe dunk is is hard coded to go directly into HP pool ignoring all effects. So effects like mana shield, dazzle grave etc are ignored. Effectively, even if the bar is full, medusa full Hp at that time of the game is not the same as say pudge hp full. One has 350hp the other has 1300 hp. The net result, is what you see. Axe one shots Dusa all game long. In fairness, the Dusa was also dumb. he waited until base was lost to figure out that he should buy bracers to boost hp. Kinda similar idea with morphling and magic damage. If morphling goes full agi, magic damage will one shot him from full hp bar (because hp too low)
