I've encountered a similar issue with a mod where the 1st item on your hotbar will be visible even when off it, and with mrcrayfish's guns the guns will appear in the same area.

Reminds me of the Cyberware and Mob Dismemberment mods. Cyberware lets you remove limbs (and organs, survivability depending on what you remove) while Mob Dismemberment lets you hack off the limbs of mobs.
