The death message "death.fell.accident.water" is a glitched achievement that occurs when you're riding in a minecart that is pushed off a cliff by a piston, causing you to die from fall damage on a waterlogged block. This uses the same logic as "player fell off a ladder". The ladder is the last block you touched, and so the game gives you a separate death message for falling off of one. Therefore, since water isn't a block, the last block you touched has to be waterlogged. You've now fulfilled the death, fell, and water parts of the message. Now for the accident part. The only way to do this is to be pushed by a piston while in a minecart. I don't know why, it just is. Gonna say smth about the fall damage ones. There’s “Fell from a high place” and “Hit the ground too hard” Pretty sure they either are random or depend on the height you die from. (High place id assume is 20+ blocks, enough to one shot at full HP, while hit the ground too hard is anything lower)
