I have an explanation for Onryo! In her lore, she is trying to escape the well by attaining power through spreading her image, (the well itself is actually very similar to the entity, created by the god of Pain and Torture. She actually can’t even exist in the Entity’s realm normally, she likely struck a deal to appear in the realm for a mutual benefit, she spreads her image and gains power, the Entity gets emotions to feed on) meaning her blighted skin is almost likely a creation of the blight. Her image is powerful, even as a simple drawing. The blight could have created an image of the Onryo (or even infected a tape) to make an image of her that was blighted, allowing her to tap into that power at will.

The Entity may have released Andy as soon as they trapped Chucky, not to his knowledge, which would mean he's spending eternity thinking he'll one day have the human body he so desires. And if Andy is still on the realm, that makes him the youngest survivor at 8 years old, even younger than Cheryl. And, as the realm is seemingly atemporal, Andy is not maturing anytime soon. The creepier part of this, is the fact that The Entity feeds off fear and hope. If Andy was released, it'd be a massive beacon of hope as his worst nightmare would be gone for good. But, if Andy is still in the realm, he's being tormented for eternity with the thought of clashing with Chucky once again. The Entity is having an entire barbecue from Chucky's hope and Andy's fear of meeting one another one more time within the realm, both unknowing it wouldn't be their last time.

Mint skull please say something about my theory 😭. The reason for the meat and distortion on maps like Borgo, rpd, and Greenville is because they are based on memories. It is already pretty well believed that the entity takes maps from the memories of the killers and survivors, but in the case of rpd it took the memories of Jill, Carlos, Leon, Chris, wesker, and like 3 other people, all of which were in the station at different times and remember it differently so maybe the entity matter is from the entity literally frankenstiening their memories together. With the Borgo the memories could be drawn from Vittorio and tarhos, but also guards. Greenville is only pulled from the unknown and sable, but something tells me that the unknown doesn't exactly perceive the world in a normal way, also while the unknown likely spent most of its time in the woods of Greenville the theatre was a very significant place for sable which could be why the map is a theatre in the middle of the woods

Even if there is evidence to the contrary, I find it a lot more entertaining to think that Pyramid Head is an anomaly in the realm. He doesn't obey per se, but he doesn't fight back either. His entire motive, iirc, is forcing people to deal with their past traumas or else. The realm is likely one of the most traumatic places in all of existence, so he's just there doing his thing. The reason the entity doesn't do anything about him is because his "thing" just happens to align with trials anyway, and the entity doesn't want to make the effort.
