This is super old, so you probably have figured it out by now, but I'm pretty sure it's "Lily Sunder Has Some Regrets" (12x10). It takes place after "First Blood", the 250th episode they were talking about, features Cas, so Misha would be on set for this interview, and the clothes fit (Dean in all black, Sam in black&red plaid). In one scene Dean cuts his palm to draw an angel sigil, which would explain the hand.
