Patient no. 2. It is a pity no treatment was given while the diagnostic workup continued. He ended up worsening with desaturation. A second CT was definitely required. Without that, a diagnosis of PF ILD could not have been made. And was an acute exacerbation (or another cause of functional deterioration and desaturation) investigated. If PF ILD is not conclusively proved, how can ninetedanib be started.

Without the correct diagnosis, there is limitation on treatment strategies while work-up is done and is not typically recommended. The key is rapid work-up and diagnosis. While we cannot rule out the relationship of clinical deterioration with surgical lung biopsy, he did not have an acute exacerbation. He did have a decline in FVC and symptomatic worsening which was sufficient to define PF-ILD (based on IN-BUILD trial at the time this patient was seen and per the new guidelines now for progressive pulmonary fibrosis)
