Very detailed explanation, helpful for Freshers/Newbie. But you have missed an important chart i.e the Risk ranking of Severity, Occurrence and Detection. All 3 (S, O,D has 1 to 10) but one should understand the standard rank based on level of risk. So the 1 to 10 should be explained. Or can be referred to any templates available online.

Permission to ask, do S, O, D always use rank 1 to 10? There are journals that use a rank of 1-5. For standard usage, should it be 1 to 10 or what? Thank you
