Well, he wouldn’t need the hourglass for that since he could stop time and do that again. But the reason why he can’t is because time is unaffected to a hellspawn, and he can’t have his humanity back because of his body count. Even god admitted to him that he made an illusion where he had the Mother of Existence/Man of Miracles tell Spawn that he hit Wanda in the stomach which caused her to miscarry her baby as an another excuse for them not to give his humanity back.

If you check the Image in the last panel aside from Spawn, Skorpion and Sub-Zero, there were other spawns standing on the cliff. He used the hourglass to travel to other timelines and dimensions to collect the alternate versions of himself (He just didn't mention using the hourglass, but with Kronika in the picture you don't really have to mention that. It's already obvious). You can see Medieval and Grim Reaper Spawn right behind him.

You know, I actually thought that Spawn ending would've involved him using the Hourglass to fix his mistakes and find redemption...but then after seeing this, I remembered the tragedy of Spawn. Much as I'd love to see him redeemed, that's unlikely to happen, mostly because he never believed himself worthy of it after everything he's done. So in a way, this fits a lot better than the ending I thought would happen.
