Since Flex was moving the shot was random, and where it went was calculated server side, but the visual is calculated client side. The clarity of it is what Riot is trying to improve on but it's not great atm.

Riot is struggling with clarity and client-side feedback. If you think your shot went in someone's head but it didn't kill them, they're confident that you're wrong because of visual clarity. But according to them, everything should be fine server-side (where the raycast is calculated) Imo, it's a big problem if people can't find how and why they didn't kill someone in such a tactical shooter, I was mad when I read it would probably stay that way for a long time.
