I have a few distant cousins who work on reality TV (one worked on OG Kitchen Nightmares) and most of the time they aren't scripted. But, contestants or people on the show are bribed to make a scene or PAs are given bonuses for causing drama. The bigger the sound bite the bigger the bonus. It could be cash or gifts, like a new wardrobe and laptops.
